Brief Answer:
Most probably you may be suffering from the bilateral SNHL

Detailed Answer:
Hi Sir,
I have gone through your query; and it suggest that most probably you may be suffering from the bilateral sensorineural hearing loss (SNHL) with tinnitus

I would like to inform you that there are so many reasons of bilateral sensorineural hearing loss with tinnitus in your age group are-
1. Presbycusis (old age).
2. Noise induced.
3. Chronic otitis media of the discharging ear.
4. Ototoxic drugs and
5. Earwax impaction.
6. Diabetes mellitus and/or hypertension.

But to reach the near diagnosis or for prescription first I would like to suggest that you may go for
1. Pure tone audiometry (already done but send me the report or discuss with your ENT specialist).
2. Impedance audimetry.
3. Blood sugar level.
4. Blood pressure measurement and
5. Otoscopy of the ear.

Either you will have to discuss these steps with your treating ENT surgeon or send me the report of above mentioned investigations for further management.
