Question: My son is 6months old. I started to notice what looked like hives on his hands. His one hand is swolen at times and looks red aswell. What can it be?

Hi

Welcome to the HCM
I have gone through your question and the attached picture. Although the pic is not so clear, I can see the redness and swelling on dorsum of hand.
Most likely you child is having allergic tendencies probably to insect bites or other allergens to which hands are coming in contact.
I have seen such cases in my clinical practice and recommend the following measures:
1. Try to get rid of the allergen if possible. It may be insect bite, soaps, baby products, food product, fabric etc.
2. Laboratory tests such as skin allergy test, IgE and AEC (absolute eosinophil count) are useful.
3. In case of any such episode, you may give Syp. Benadryl or Chlorpheniramine. These are antihistamines and help in resolving the reaction.
4. Use Lactocalamine lotion on the affected site and keep it elevated by pillow for early relief of swelling.

In case there are any other symptoms such as whole body urticaria and breathing difficulty, immediately take the child to ER for emergency treatment such as adrenaline and steroids under proper doctor supervision.

Why does this not show on the rest of his body? Only his hands are red and swolen.

Probably because hands are usually uncovered and come in contact

Detailed Answer:
Allergic reaction can present in various ways. This presentation may be because the hands are usually uncovered and come in contact with allergens.
Since it comes and goes, so the other possibilities such as lymphoedema, hypothyroidism, congenital anomalies, cellulitis etc. are less likely.

Do not worry, the measures I recommended earlier should work.
