Question: Hi, I had a bad fall 10 days ago when I fell flat on my face. I developed a large "aubergine" black and blue on my lower left abdomen which does not bother me at all. This morning after urinating I noticed "old" blood on the tissue. Could this be a result of the trauma? I have a doctor's appointment next week.

I fell flat face on the hard floor and have large bruises on my lower left abdomen ,left knee and upper right arm
Blood from vagina - purple tinge - never before has this happened
No it doesn't hurt to urinate.

I am sorry about your fall and I am glad that you are overall okay. Given the injuries that you are describing, it sounds like you hit your face, abdomen, left knee and right arm. My guess if that you might also be prone to bleeding as many women your age are at least on a baby aspirin. The bruises are normal. Given the bruising on your lower abdomen, it would suggest that you had a blow that might have been close to the bladder. The bladder is right above the pubic bone. The inside lining of the bladder is prone to bleeding in some cases and this could be related to the fall. The other source of the blood is from the vagina, but this could be difficult to sort out. For right now, it sounds like you are okay to wait for your visit to sort this out, but I would suggest that you make sure that the doctor addresses the following questions:

1. First, they need to make sure that it is clear why you fell. Is it that you just tripped on something, or did something else happen (like a balance problem for example)
2. They need to consider getting a urine sample. If there is any persistent blood in the urine, then there might be something in the bladder that is prone to bleeding. Perhaps the fall brought some underlying problem to light.
3. If the bladder is not the source of the blood, then they need to consider GYN sources of the bleeding, such as the uterus. This could easily be sorted out with an exam and an ultrasound
4. Finally, it is worth making sure that you do not have some type of bleeding disorder. If you did not really hit your belly but have a big bruise there, then you are somehow more prone to bruising and this also might be a sign of an underlying problem.

Thank you. There has been no further blood for over 24 hours. I think I may have injured a vein or something internally as that bruise is big but healing nicely.
