Posted on Sat, 15 Nov 2014
Question: I have had neck and back pain for years and have had nerve pain periodically during a flare. Luckily MS has been ruled out. I have psoriasis and am wondering if I have psoriatic arthritis. A flarw often occurs after I have been sick. The fatigue and pain can be debilitating at times. I fight it because I love to work out and exercise. I have not been able to bike to work since XXXXXXX and I have reduced my swimming to 1 day per week and have bought a training snorkel to reduce neck movement. I go through periods of feeling well with man eagle pain and then flares. I have two boys 10 and 7 and would like to nfind out what is going on. Do you have any suggestions?

Brief Answer:
Psoriatic spine disease is most likey.

Detailed Answer:
Hi Madam,

I have carefully read your query and understand your concern.

As per your symptoms and clinical history ,a high likelihood of Psoriatic arthritis-PsA is possible in your case.

PsA has good degree of propensity to affect the cervical spine.

Psoriasis flares are well associated with arthritic symptoms and features systemic illness.

In order to detect the flares clinically, lab tests like ESR and CRP are quite helpful in your case.

I strongly recommend you to have an MRI with gadolinium enhancement of the cervical spine to accurately document the arthritic changes at the earliest.

PsA is known to cause only mild joint erosions in most cases.

Definitive treatment protocols do exist for the cervical spine disease once the diagnosis is made certain in your case.

Post your further queries if any,
Thank you.
