What Causes Nasal Discharge After Surgery For Head Injury?

Question: Hi Doctor,
I meet with accident on 2013 April and had surgery for Head Injury. Also i had CSF Rhinorrhoea for left noise for nasal discharge. I didn’t have nasal discharge for one year. But after year discharge started in left noise and discharge is more than last time. I had attached my medical report for my surgery last year. Kindly let me know how what is work around to stop this discharge.

Hi,

I have gone through your question and understand your concerns.

As per your reports, you had successful surgery (frontal craniotomy + CSF leak closed with temporalis fascia) for a year. In my opinion, present leak could be from the same site or different site.

You need to undergo CT with contrast or MRI again to relocate the fistula (csf leak).

Depending on the site, size of fistula one can decide the treatment modality.
Do not worry, this time you do not require major surgery, can be managed by endoscopic surgery to close the fistula.

I advise you to continue antibiotics to prevent meningitis (common in csf rhinorrohea patients) kindly do the investigations.

I have few query regarding from your answers.
1) what could be the reason for my problem recurring after one year?
2) I am not taking any antibiotics now and it is been for more than one year I have stopped the any medications.
3) if I need surgery to fix the problem, how many days rest required after the surgey?

Solutions for your queries.

A.Reasons for recurrence - 1. Raised intracranial pressure happens in obese people or some times iatrogenic. 2. Fistula should be closed with bone graft supported with the muscle.


B. You need to take antibiotics, direct communication out side environment (nose to to brain) can lead to brain coverings infection leading to meningitis.


C. Around 7-10 days of post operative period.
