What Causes Low Grade Fever Every Evening?

Question: My husband runs a low grade temperature every evening but he has no specific pain or anything to give him a reason for the temp and none of our doctors has no idea what could be causing it. There is a lot of Cancer, Heart Problems and Diabetes in his family so he is really concerned as well as I. Can you help us?

Hello,

a prolonged (?) low grade fever is difficult to investigate if no symptoms or signs can lead the diagnostic thought to a certain direction. If his doctor could not detect any clinical signs during clinical examination then some basic tests have to be done to search for hints.

Basic blood tests include: ESR, CRP, complete blood count, urea, creatinine, liver function tests, urinalysis. If no useful finding occurs then radiological tests might be required. You can start with lower cost tests like a simple chest X-ray and an ultrasound scan of the abdomen and continue with tests like CT-scan of the abdomen (and/or the chest).
Other possibly useful tests include endoscopy of the upper and lower gastrointestinal system.

Depending on the results, other tests might be required. You should understand that this kind of symptom is too vague and the doctor has to take one step at a time. The findings may change the strategy of investigation.
