It seems to be some fungal infection associated with secondary infection.
Don’t use dettol for cleaning the area. It will cause further irritation.
Wash your inner wears with any antiseptic solution(dettol). It will avoid further contact of infection.
Don’t use inners during night and while you are at home and rest.

As you need blood purification. I will prescribe medication kindly continue it for one month.
Ayurvedic medication:
kadiraristam-20ml with 60 ml of warm water twice a day.- for one month.
Allerin tab- allarsin company- 2 tab thrice a day
Charmaraksha ointment- AVP pharmacy- ayurvedic medicine. Apply it on affect part. Apply it twice a time. During bath use normal separate soap to clean the area.
Neem capsule- Himalaya ayurvedic drug- 2 cap twice a day.

Avoid Junk and fried items. Eat more vegetables and fruits.

2. Allerin tab- allarsin company- 2 tab thrice a day. (Morning 2 tablet, afternoon 2 Tablet and night 2 tablet- after food.)

2. Neem capsule- Himalaya ayurvedic drug- 2 cap twice a day. (Morning 2 tablet and night 2 tablet- after food.)

3. Himalaya antiseptic cream- external use and Charmaraksha ointment- AVP pharmacy- ayurvedic medicine... Mix both the cream and ointment and apply. Apply it on affected part. Apply it twice a time
During bath use normal separate soap to clean the area.
