What Causes Heavy Menstrual Bleeding With Clots And Moodiness?

Question: I have been going through some very intimidating symptoms and am fairly young, however I have been having very heavy menstrual cycles, with large clots… some larger than a quarter. When the clots come I can feel them, like a contraction. My periods lasts for at least 7-10 days at a very heavy flow, so heavy that I soak through a super tampon and pad within 30 minutes now. My body feels like it’s on fire sometimes and I’m very moody. I have had ultrasounds and blood tests and every doctor seems to think it’s all in my head…. They have placed me on meds to help slow the bleeding and deal with pain however hasn’t stopped… I still get clots and heavy periods. I even started taking birth control again to which I stopped after a month as I didn’t stop bleeding the whole time I was on them…. Changing my tampon and pad every 30 minutes. I’m only 33 however my mother went through menopause when she was younger then I am and by my age a hysterectomy. I don’t know what to do anymore and no doctor seems to listen when I say is it possible…. Is it?

also I have gained weight and starting to get small chin hairs??

Thanks for the query to hcm,
The condition that you are experiencing in terms of heavy menses is termed as menorrhagia or excessive menstrual bleeding. There can be various causes for the same and each should be evaluated. There can be uterine causes like uterine polyps or submucousal fibroids which can cause irregular heavy menses or there can be primarily progesterone deficiency for which you would have to be placed on cyclical progesterones for at least 3-6 months.
But first you should get an ultrasound pelvis done especially a transvaginal ultrasound to rule out any uterine polyp or submucosal fibroid , if found then a hysperoscopic removal shall solve your problem to a great extent. Also opt for a hormonal profile including thyroid and prolactin levels along with serum progesterone levels. Thirdly get an endometrial biopsy done for histopathological examination to rule out any endometrial hyperplasia which can be another cause for your heavy bleeding. Another possibility is ADENOMYOSIS which is a form of endometriosis which is again a very common reason for menorrhagia with dysmenorrhoea. In such cases usually hysterectomy or GnRH analogues or continuous progesterone pills for 6 months usually helps reduce the pathology.
I would like you to get back with these reports for purview. In the meantime as you are not on any contraception, practice barrier contraception and for reducing the flow you may take antifibrinolytics like TAB TRANEXAMIC ACID 500mg thrice day for 5 days from the day 2 of your menses to reduce the flow along with an NSAID to reduce the pain. These are available on prescription.
So you may discuss the options with your physician and take an informed decision.
