What Causes Heavy Bleeding, Lower Back Pain, Bubble Like Sac During Menstruation?

Question: Has found a bubble like sac during my period 2 days ago, had heavier period than normal and huge clots, my abdomen hurts and my lower back, its my 4th day and I still have cramps, back pain and bleeding with huge clots. I dont know what to think. and I was delayed for over 5 weeks but when I did took a pregnancy test it came out negative.
