Question: hi my name is XXXX i am 57years old and have an inlarged prostate i am on flowmaxtra but still having problems my doctor is reluctant to send me to another specialist i live in country nsw my psa is aparently around 13 and prostate is 57+52+67 is this normal to be going to the toilet as many as 30 times a day and 3 to 4 times a night

Hello XXXX .
You are taking flowmaxtra for enlarged prostate and it seems that it is not working because frequency of urine is 30 times in a day as well as 3-4 times in the night.
PSA is 13 ( normal range is 3-5 ng/mL ) ,in your case the level of PSA is very high and this is at dangerous level .
The size is 4X2X3 cms ( in your case it is 57x52x67 mm ) .This shows that there is enlargement of prostate .
Frequent urine may be due to enlargement of prostate.
The reason of frequent urination may be due to these possibilities , these include:
1 Enlargement of prostate , as diagnosed is confirmed that there is enlargement of prostate but in your case the level of psa is too high so I recommend you please get in M R I , serum acid phosphatase test to rule out the malignancy of prostate .
2 Prostatitis ( acute cum chronic ) , since you didn't mention the duration of symptoms so some times inadequately treated acute prostatitis turn into chronic cases .Diagnosis can be confirmed by urine examination for prostatic threads , rectal examination ( physical by surgeon) and urethroscopy .
3 Benign enlargement of prostate , clinical history and physical examination is important in such cases .
In your case psa is very high and size is also large so I recommend you please consult an UROLOGIST and get his opinion at earliest .

hello dr bishoi i have had symtoms since febuary this year have had mri 19/5/2014 results no cancer shown urologist said 6 months recheck wondering if to get second appinion to see what else can be done if anything my problem is i am 500kms from melb 800kms from sydney and no health insurance thank you for your help XXXXXXX i have also had biopsies no cancer but the uologist said there were areas he could not get to and there could be cancer there but he did not think there was any cancer my problem is uncertince ofwhat todo next thank you

Once again ,thanks for the query.
As biopsy proved that you are not having cancer ,so you can be treated easily .
Now two possibilities are there:
1 Benign prostatic hyperplasia , when such patient visit and don't respond to flowmaxtra , I prescribe them to take a combination of TAMSULOSIN+DUTASTERIDE one tablet once daily . Since at present there is no retention of urine and you are already taking tamsulosin, so this combination will work within 7-10 days and you will get relief .
But take this treatment for at least 12 months .
Following treatment is blind treatment when facilities for urine culture is not available .
2 The next reason may be chronic prostatitis, so take a broad spectrum antibiotics as TETRACYCLIN 500 mg 4 times in a day for 10 days or till urine culture report ( if facility of urine culture is not available take tetra ).
If culture facility is available then after culture report start treatment .
In such case the drug of choice is SULPHAMETHIZOLE 100 mg 4 times till 15 days and this is also to prevent recurrence .
As I have noted that if once patient get relief from urgency (frequency) of urine keep tamsulosin+dutasteride, treatment for 2 years .
Discuss about these medications with your doctor.
