What Causes Decreased Hemoglobin Levels And Raised Ferritin Levels?

Question: Dear Sir, I have asked this to several doctors and I want to have your opinion as well. I'm a healthy person who sports 4-5 times a week and eat very healthy. Lots of fish and chicken and less meat. 4 years ago I have had a blood test and came out that I had a high gamma gt (80 and normal was 50 as stated and My ferritin was 257 as I remember with a HB of 9.2) and this was due to my diet and alcohol consumption as well as light smoking. I was referred to an ultrasound and it came out that I had a light / mild fatty liver. Also my cholesterol was a bit high. so I decided to change my lifestyle, so I started work outs and running for 4-5 times a week. I lost 10 kg and became 75 kg from 85 (I am 171 cm tall) and all values came normal since then. My cholesterol, lever .. etc.. I check blood every year and for this year something weird came out ( at least I think weird) My ferritin is 324 while it's stating on the report 17-230 for men.. My hb is 8.4 while it had to be minimum of 8.5-11 (but maybe this is because I sport too much?). I was suffering mild cough a week or a week and a half before the test . I also had lots of anxiety and stress the last time. Also I have eczema and nail fungal infection on foot. ( just for information to help you giving your opinion). so if this high Ferritin was from this condition, would go up quickly and then drop down slowly? I will attach my report as all other test came out normal. Crp, liver values, thyroid, Glucose, white and red blood cells...etc.. all came normal but perfect. Do I have to worry much about this value of ferritin? I have my fears and scares for this now. Note that there is no known family history (father, mother) history of hemochromatosis in the family. I don't eat much red meet as I eat fish almost everyday in the afternoon and most of time chicken in the evening. One more update, I'm consume alcohol in the weekend.. Mainly red wine. Like 5-9 glasses of wine in one weekend Value of ferritin indicated is in μg/l Can the mild cough and or stress could have caused the high ferritin while CRP is normal value? how quick does Ferritin and CRP raise due an infection and how quick it does reduce? I have been told that stress can reduce the HB a little bit and also result some higher ferritin. My gp wasnt worried about these results and because I was worried he suggested another test after 3 months. The last couple of weeks before test I didn't eat well, could HB get little lower in 2-4 weeks because of bad to less eating? I await your answer and information and sorry for this mixed information. Thanks for your time, regards, XXXX

Brief Answer: Investigations suggest anemia of chronic disease Detailed Answer: Hello, Thanks for posting your query on Healthcaremagic. As per your information, all blood tests, liver function tests and cholesterol level are within normal limits. The only abnormality is decreased hemoglobin levels and raised ferritin levels. Low hemoglobin is called anemia and you are suffering from anemia. The cause of anemia has to be found out to treat anemia. Raised ferritin level is seen in anemia of chronic disease. Anemia of chronic disease is a very broad term. Anemia of chronic disease includes various diseases of different organ systems. Diseases of various organ systems leads to suppression of hematopoiesis. These diseases include renal diseases, liver diseases, thyroid diseases etc. You need to consult your physician to find out the cause of anemia of chronic disease. If you have more queries, I will be glad to answer. Dr Shailja P Wahal

Brief Answer: Other possibilities of anemia have to be looked Detailed Answer: Hello and welcome again, The low hemoglobin is definitely due to anemia, Low hemoglobin with high ferritin levels suggests some underlying disease. Thyroid disease is one of the diseases which can cause anemia of chronic anemia. Other possibilities have to be looked for to to find the cause. If you have more queries, I will be glad to answer. Do you have some information about inflammation or infection and acute protein reaction? I am writing you something about the cough and how could that be affecting? Also the last month because of anxiety I didnt eat well.. And had lots of stress... Doesnt stress react like infection? It does something with the immune system or am I wrong? To be honest, I now asked several GP's and one specialist. They didnt mention a huge concern. I have never had chronic anemia. I have never had anemia at all. This is the first time. as I wrote, back in 2010 I had a hb of 9.2 and my ferritin was 257 which was high as well.. Since then my gp didnt order the ferritin test for me.. I have checked my HB now for 3 times after 2010 and all values were good. Last one was 8.7 some websites are stating 8.1 as a border of anemia.. I still have 8.4 !! YOu already have made your diagnosis basis on this information? That I have an anemia of chronic disease? I will attach my report as it maybe will help you reviewing your answer? Thanks for understanding. I just remembered something . I have fungal nails after a long duration fungal infection on my left foot.. 4 nails are infected. Could this be something which effects ferritin?

Brief Answer: Fungal infection will not affect the ferritin Detailed Answer: Hello and welcome again, The fungal infection of the nails has to be treated by anti-fungal drugs. The fungal infection of the nails will not affect the ferritin levels. You need to consult a dermatologist for the fungal infection. Looking at your report it is evident that hemoglobin level is low and ferritin level is high thus, this picture is suggestive of anemia of chronic disease. Consult your physician with the report. After another consult with a Gastroenterologist here she told me that fungal nail infection Is a kind of chronic infection where ferritin can react on. I have consulted several gps and they reassured me that I don't have to worry and just redo the test in 3 months. They also told me that it might even be die to stress and the body reacts on stress as inflammation like.. Which can decrease the hb a little as well. They also were not concerned about my hb at all. As it also might be due to sport I do, where the blood will continue more plasma and become lighter, so it might be a false anemia.. You didn't inform me how quick the ferritin can go up.. And how quick it drops down if it was due to acute infection ? Does it react else than crp? How does crp react? Sure if you know and are sure about the info. If not then it's not important but just let me know. Chronic disease is in my case a little strange cause I have tested for hepatitis and hiv 3 years ago and haven't have any risks after the test.. Also I have no chronic colon or stomach disease as there were scopies done 2-3 years ago. My liver was also screened by ultra sound 3 years ago and came out with a light fatty liver. Probably from alcohol. My thyroid and kidney values are perfect here. I have no symptoms at all. I feel good and have a very good condition. I hope I have been clear in the information given and if you have any question just let me know.

Brief Answer: Prompt Tt of fungal infection Detailed Answer: Hello and welcome again, Hemoglobin of 8.4 is low for your age. The fungal infection should be treated promptly. CRP increases in infections and inflammations and its value decreases quickly as the infection/ inflammation subsides. You can get the hemoglobin and ferritin levels repeated after a month or so. Fungal infection of the nail takes around 3-6 months to clear completely so start the treatment as soon as possible. Thanks and take care Dr Shailja P Wahal

Dear As per report.. Hb in the lab I have done the test states (8.5 - 11) and it's deferent from lab to lab as I am told. So it's less in 0.1. But you don't go in details of sport and hb.. Also how ferritin reacts.. You answers are too general and doesn't go in details in my case.. Also I wanted to know if ferritin acts exactly like crp? And if ferritin elevates if crp elevates or not.. But thanks a lot.. Just wanted to thank you and I will close this case after your answer My gp didn't advise me to treat the nail fungal as it can raise the liver values .. So I'm thinking to treat it by laser.

Brief Answer: Liver function tests need to be done to look for liver status Detailed Answer: Hello and welcome again, There is a history of fatty liver disease. The gamma GT levels are raised. What is the status of SGOT and SGPT? Anti-fungals are metabolized by liver and kidney. Thus, these drugs are avoided in liver and/or kidney disease. Treatment of the fungal infection will depend on the status of liver and kidney. Consult your physician with reports. Fatty liver is reversible initially. Requires abstinence from alcohol. Gamma GT was raised back in 2010.. But now not any more. You didnt look to the report I attached, please have a look. The alat and other liver values are there. All normal.. fatty liver was very mild and it must be released by following the diet and sport as the gamma gt was low again. my kidney as well as thyroid function are stated on the report as well.

Brief Answer: LFTS should be repeated in case of antifungal drug Detailed Answer: Hello and welcome again, You have mentioned that you want to take laser treatment for fungal infection. In case you want to take anti-fungal drugs, do take up liver function tests before that. Rest is fine. Thanks and take care Dr Shailja P Wahal

doctor, i was consulting you for my ferritin. This is my concern. Not my fungal. I wanted to know your opinion. You finished by diagnosing me that I have a chronic infection.. So my fungus nail is a chronic infection which can raise the ferritin then.. or not? And you also said that I have anemia of chronic infection (anemia when having a 0.1 less than the value required). Well.. would ferritin be this low? and why crp is 1? so normal..

Brief Answer: CRP comes back to normal earlier than ferritin Detailed Answer: Hello and welcome again, After any chronic infection/ inflammation in the body, CRP comes back to normal soon but ferritin takes a little longer. The liver function tests are within normal limits, you can get ferritin levels again after month or so. Thanks and take care Dr Shailja P Wahal
