Read and reviewed your query with medications tried report.

Seems that though his chest ribs are normal,he seems to have had -Intra-abdominal Contusion injury,which needs to be assessed and evaluated by-
a-Standing X-ray Abdomen with domes of diaphragm-to assess gas under diaphragm.
b-Consults with Endoscopic-GI Surgeon-who would evaluate with Upper GI EGD (Esophago-gastro- duodenoscopy examination and Lower GI-Colonoscopy-

c-Would need to rule out- gastric tears-causing dark stool today from sudden pressure injuries from seat belt. Other injuries accompanied like soft organ lacerations/contusions-like liver/ spleen,need to be ruled out.

d-Mesenteric Vessels thrombosis -from abdominal contusion also needs to be rule out,for his advanced age.

e-USG abdomen and CT abdomen to assess bowels and soft organ is suggested for proper evaluation, before planning any further treatment for his worse condition today.
