Question: My daughter age 40 yrs had a tummy bug/virus in January this year, and has not been well, or been able to work since. She is constantly nauseous, eats little, and has lost 2 stones in weight. Some days she just lies in a darkened room all day. She has endless tests, blood/stool /urine/vitamin levels. MRI of small bowel/ultrasound of gall bladder/CT brain scan/ and endoscopathy of stomach and large bowel. Nothing has been found to cause her sickness in any test or scan. The latest thoery is that a virus has damaged the vegus nerve, or the start of MS in an unusual site. She was an active healthy woman who rarely came down with a cold or flu, in a busy interesting job that she loved.Have you any idea what could be causing this. She is now becoming very depressed, but she is unable to keep antidepressants down. Many thanks, XXXX

Dear Dr Saha, I regret that I do not have access to my daughters tests and scans ect.
They have been done so far by 2 GPs, 2 gastroenterologists, 2 dieticians,and an alternative doctor.We have appointments with a ENT specialist and a Neurologist later this week. All tests and scans have been 'normal' so far, but I don't know what the neurologist will do in the way of tests. We are trying to find a Virologist, but I do not know what tests he can do either. XXXXXXX is 5ft 8' and weighed about 10stone 6lb before her illness. She is now about 8stone 4lb. Do you have any idea what could be making her so ill? XXXX

Thanks for writing back. I understand the results were normal but still perhaps a correlation could be drawn between the individual parameters had they been available. I would also have liked to know specifically the Chest X ray report since that could help me exclude certain chronic granulomatous diseases that could be linked to severe weight loss as in this case and based on the findings it could require further correlation with certain other blood parameters. I would also have liked to check the blood counts and look into the specifics of the differential leucocyte count and ESR. I would also have liked to read the specific values of the C-reactive protein or procalcitonin. I would also have needed to know specifically the value of albumin and globulin in the LFT report. Based on these reports I might ask for a viral screening profile as well, if that has not been done already. It is hard to comment without being either able to glance at these details or examining her clinically. I have some speculations but medicine is based on correlations rather than speculations.

Instead of searching for a virologist, you should go to a infectious disease specialist if you seek a specialist opinion in the field.
