What Causes Constant Headache That Starts Behind The Neck And Moves Up?

default
Posted on Thu, 21 Apr 2016
Question: Consistent headache for two weeks.

Small clinical history:

Female patient of 37 years old
Two weeks ago she started a headache that increased when she stood up or made sudden moves.

We tried regular pain killers, but didn't worked, we did a complete blood test and found a linfocitosis, after some advice here, we gave her carbamazepine, 100 mg / day, and that helps the pain, but doesn't dissapear. Besides the headache, there is no fever, no diarreas, no vomit or nauseas, a constant headache that starts little behind the neck, and moves up.

We did another test and blood results are consistent, relative linfocitosis by an hemologist, monocitosis has gone, buy linfocitosis persist in 60%.we are suspicious of something about the thyroid hormone, any help would be appreciated. DR. XXXXXXX neurologist here has been helping so far.

Brief Answer:
Could be due to sinusitis

Detailed Answer:
Thanks for asking on Helathcaremagic.

I have gone carefully through the details. However it is hard to conclude without further details being available. I would need to know the specfic leucolyte count. The mere information of lymphcytosis (you had misspelt) is not enough. Usually it occurs in a viral infection and it is possible that she has a sinusitis and hence is getting the headache. I assume that there was eosinophilia as well, for which carbamazine is being given. Please check if it is diethyl carbamazine or carbamazepine, if the latter please mention why it is being given. Mention if she has any episodes of seizures.

An X ray of face - Occipito mental view should throw light on the possibility of sinusitis. Steam inhalation should help. Also give her some antiallergic like cetirizine once daily.

If nothing comes up and no relief is obtained, a CT scan of the brain might be needed.

Thanks Dr.

Perhaps more information is helpful, we have done several tests already, now attached, sorry for the misspelled words, I'm from Guatemala and here we spell it with an I instead of ymth, my mistake.

We strongly believe it's not sinusitis related, she has a general pain not too high on the left side of her body when the pain on the head has it's higher peaks, we tried some pain killers, basics, but didn't do anything, she was treated with carbamazepine, only, as prescribed by a surgeon, family doctor, so far nothing has really made this pain in the head dissapear, as mentioned before, the pain is a constant pressure on the parietal section of the head, sometimes the neck is part of the pain, the pressure on the head is general, the pain in the rest of the body is like neuralgia, pain spots on left leg, left arm, and recently the toes. There is no fever, no diarrea, no nauseas, just a constant pain, only relief by carbamazepine, but not dissapeared.

Please let me know if you have any questions. Also, are you able to see my other inquiries before this?

Again, thanks for the assistance.

Brief Answer:
Two investigations suggested. I understand that she is not getting relief.

Detailed Answer:
Thanks for writing back. I have gone through the additional details provided. I would still insist on getting an X-Ray of the skull-OM view done. The next step would be to get a CT scan of the brain done.

Yes, I am able to go through your previous queries although I have not done so.

Let me know if you have anything specific to know from me.

Thank you, we will follow your suggestions, please, if possible don't close the question in two days if you don't hear back from me, going to make this exams is expensive here, and it takes time, I will return when done and upload the results, otherwise it's hard for me to jump from specialist to specialist every time.
