Clinical picture: Black skin Eruption in both legs below knees pulling type pain in legs, Dryness of legs
Date of Admission:28 Oct 2015 to 24 Nov 2015
Treatment:
Vata kaphahara chikitsa by rooksh swedhana XXXXXXX swedhana and samana Therapy
Kriyakarma done:
1. 1.     Dhanyamladhara(dry)
2.     Dhanyamladhara Tailam
3.     Abhyangam Kashaydhara
4.     Lepam
5.     Thakradhara
6.     Kashaydhara
Kindly confirm whether patient with above clinical picture & treatment given require In patient treatment or can be managed on OPD basis.

Dear
Thanks for contacting Ayurveda with your health concern and after reviewing your case history I'm of the following opinion:

1. Dry skin is a complication of prameha due to high blood glucose which take away moisture from the skin and also people with diabetes have poor circulation
PS. he must be suffering from diabetic dermopathy

2. Do let me know about the blood glucose levels [before and after] and also provide with the snapshots [if available] to see the effect of the medicine(s) cum therapy for further explanation

3. Although you can very well manage the patient on OPD basis but as I don't have any direct interaction thus your attending physician will be in better position to clear your doubts, however if the patient is healthy and blood glucose value normal he can easily be managed as and when required, but with following instructions:

CARE OF THE FEET
- wash the feet daily, dry them, inspect them and then sprinkle lot of talcum powder
- avoid walking barefoot, even at home
- wear soft cotton socks & canvas shoes. Avoid tight fitting shoes 7 chappals. Use new footwear with care to avoid shoe bites
- cut nails very carefully and always after bath, when they are soft

CARE OF THE SKIN

- moisturizing lotion to keep skin soft
- talcum powder between skin folds
- use only topical application, oral not recommended
- avoid hot baths/shower which can dry the skin
- maintain optimal level of humidity [in dry months]
- avoid scratching dry skin which can create lesions and infection

PS. In Ayurveda each patient is different so please consult your doctor before changing diet/exercise/lifestyle adjustments.

RECOMMENDED INVESTIGATIONS DURING FOLLOW UP

- check urine sugar, 2 hours after lunch [once a week]
- blood sugar [fasting and post prandial]: once in 3-6 months, more frequent is advisable if diabetes is unstable
- lipid profile
- Electrokardiogram once a year
- urine for albuminuria
- retinoscopy: once a year

Thanks for reverting back

1. As of my opinion cutaneous manifestation in people with diabetes mellitus is asymptomatic and doesn’t lead to morbidity, as such doesn’t require 24 hour treatment [unless complicated with infection: bacterial/fungal infection, gangrene or complication: neuropathy] thus one can manage the case on O.P.D. basis, however you should first consult with the attending physician regarding the same.
