What Cause Abnormal Vaginal Bleeding After Taking Morning-After Pill?

default
Posted on Sat, 16 Jan 2016
Question: Morning after pill and pregnancy - question for my daughter - she took the morning after pill, and a few days later she started bleeding. The day after she started bleeding, she had a blood test, as it is suspected that she has thyroid and/or hormonal issues. A pregnancy test also formed part of the blood test. It came back negative <2. Is there any chance that she could be pregnant? Her bleeding went for 7 days, and her real periods were the day after the bleeding stopped, but they didn't come. She is looking to go onto contraception, but wants to be positive there is no chance she could be pregnant. Thank you.

Brief Answer:
Nothing to worry

Detailed Answer:
Hello mam, thanks for trusting health care magic.
I understand your anxiety about the issue but there is nothing to worry.
This abnormal bleeding is because of morning after pill and is common and not worrisome.
Ask her to switch to regular method of contraception instead of emergency pills.
Take care

Dr Manisha

Thank you very much for your prompt response.
My daughter suffers from depression/anxiety and has been self harming (cuts herself or punches walls) every time she has her period, the past 6 months. It is consistently each time she has her period.
She is currently being tested for possible under active thyroid issues, but I also think it is best to go onto contraception. Which would be most suitable for her condition? One Dr said oral contraception will balance out her hormones and this will help her low mood during her period, but other Dr said Depo (3 mth injection) is more suitable? My understanding is Depo injection is progesterone hormone, so will this have negative effect for her hormone levels?

Kind regards

Brief Answer:
Oral pills advised

Detailed Answer:
No mam she has severe premenstrual syndrome and only oral pills will help her so don't take depo shot.
Plus in my opinion she needs sertaline tablets along for a few weeks but that is a prescription drug so discuss with your gynecologist before taking the same.

Hi, thank you for your advice. She is already taking anti depressant "Lexapro" so should this be ok to continue? Also, is there a preferred oral contraceptive? I have read a few forums whereby some people experience exasperated depression symptoms on certain oral contraceptives.

Thank you

Brief Answer:
Continue taking it

Detailed Answer:
Lexapro is good and safe antidepressant and thus she can continue to take it.
Contraceptives can cause depression in certain people but the incidence is only 2-3% and beneficial in rest.
Take the contraceptives with drospirenone as a progesterone component as in intimacy pills and crisanta LS pill strip.

Thank you so much Dr XXXXXXX for your advice. I will ask for that type of oral contraceptive pill. I have also attached a copy of her blood test. Could you please review the hormone component. LH seems high, and endocrinologist on this site has suggested further exploration around possibility of poly cycstic ovarian syndrome? Could you please advise your opinion? Also, if LH hormone is too high, will this make more problems if she starts oral contraceptive?

Warmest regards

Brief Answer:
No reports attached

Detailed Answer:
There is no report attached to this mam. You have a feature to upload the reports by yourself on the right side of this query page. Please utilize it. Alternatively, mail it to  YYYY@YYYY subject line as 'attention to Dr. Manisha Jain'.
In general if LH is high it is usually in PCOD and thus needs active management and early start of family is always desired but first control her cycles well with these contraceptive pills for a couple of months.
Please upload the reports so that I can advise you better.

Hi Dr XXXXXXX

Thank you so much for your on going assistance with my daughters health issues. At the present time, she has not done any follow up or arranged appointment(s) with any specialists, so it is very good to gain some understanding of her possible health issues, and pass this information on to her. I will try again to upload the blood test report. Our local Dr advised there is no abnormalities with the results, however I would like your opinion? I believe the LH level appears high, and I am also concerned as TSH is 1.0, however a one month earlier, the level of TSH was 6.1.

Warmest regards,
XXXX

Brief Answer:
Weight reduction

Detailed Answer:
Most welcome mam.
Yes her FSH and LH levels are definitely high and LH and estrogen are very high.
I would like to know the day of her periods when these tests were done because best results are seen if blood sample is given on day 2-5 of the menstrual cycle.
With these levels she can have problems with conception in future.
But for now she just needs weight reduction in case she is overweight.
Take care

Hi Dr XXXXXXX

The blood tests were done - 7.12.15
Bleeding started - 6.12.15
Morning after pill taken - approx 3.12.15
Periods were due - 15.12.15 (periods did not come)

She is on no other medications which could affect the results, except for Lexapro (antidepressant), which is 10mg daily. Tests were done in the morning after fasting.

Thank you

Brief Answer:
Urine pregnancy test advised

Detailed Answer:
Hello mam, nothing to worry as it is common to have delayed periods after emergency pill intake.
But to be on safer side do a urine pregnancy test.
If it is negative then just wait for periods and nothing else needs to be done.

Brief Answer:
Chances of pregnancy still negligible

Detailed Answer:
Yes mam, I do remember that but as I told you this time also this is most likely due to the emergency pill intake but asked you to do pregnancy test just to be double sure.
