Suggestt Reatment For Flank Pain Due To End-stage Renal Failure

Question: Hi, I have end-stage renal failure, single right kidney, eGFR of 10. Left kidney removed 14yrs agondue to pylonephritis. Been a kidney patient for 20yrs. I am experiencing severe pain in my right flank, have taken 50g pethidine to no avail. No bleeding, burning on urination, no kidney-stone spasms. Am I ok to wait till Monday to speak to my renal team or should I do it now? I dont want to be admitted to hospital as I am scared. Had a fistula created in August.

I appreciate your concern

Since you have a low GFR and have already got a kidney removed, nothing should be left to chance. You should definitely contact your renal team , now.

Even though according to your description you don't have any bleeding, burning on urination, or kidney-stone spasms, Right flank pain can occur in many conditions including UTI, kidney/ureteral stone, Gynecological problems. Pyelonephritis is also a possibility if an infection is present along with any obstructive component along the urinary tract. It may cause the GFR to fall suddenly and deteriorate the condition more.

Its understandable to be scared going to hospital but the benefits outweigh the risks. You should get a Urine R/E , Ultrasound abdomen, Blood counts ( to look for a raised TLC or neutropenia ) , to rule out the above concerns.
