Question: Hi. I am quitting Zypadhera (olanzapine). I think i am starting to have withdrawal symptoms. i need some questions answered :) thanks alot :)

I have been on olanzapine for almost 2 and a half year.

I was diagnosed with schizophrenia which is why i started on olanzapine.

I am on like 7,2 milligrams per day today.

I am in the process of quitting this medication (olanzapine)

I need to know:

1. My psychiatrist told me that smoking makes the drug go faster out
of the body. Do you know how much faster smoking makes olanzapine go out of the body ?

And is it all tobacco products that make olanzapine go faster out of the body ? (i use alot of nose tobacco and take tobacco in my lip and smoke cigarettes)

or is it just smoking ?


2. I was on "needle injection type of olanzapine (Zypadhera)" 210 Milligrams per 28 days. If i completely stop taking those needle injections in one day (do not stop slowly (reduce the dosage slowly until i am only taking maybe 1,25 milligrams per day) then does the olanzapine slowly leave my body in a steady and regular and even pace until it is completely gone out of my body ?

And how long from the day i completely quit and until it is completely gone out of my body ?


3. I am in the process of quitting this olanzapine drug. Last time i took the drug was 34 days ago (then i took a 210 mg dose of needle injection olanzapine (Zypadhera), 28 day dose.)

I have not taken anything since then but i plan on getting tablets of olanzapine and take 5mg per day tablets and then slowly reduce the tablet dosage until i am completely off the olanzapine drug. What are the 3 most important "things or some" that you can tell me to make this coming off of the olanzapine drug the safest road for me ?

I want to quit this olanzapine medication very safely. Please tell me how what i need to know to quit this drug very safely.

You are on olanzapine depot injections. You are taking 210 mg dose every 28 days(4 weeks). However it does not simple mean that it is 7.2 mg per day and after 28 days there will be no release of drug.According to different pharmacokinetic strides this dose roughly provides 5 mg per day equivalent oral dose.After a single IM injection with ZYPADHERA, the slow dissolution of the olanzapine pamoate salt in muscle tissue begins immediately and provides a slow continuous release of olanzapine for more than four weeks. The release becomes diminishingly smaller within eight to twelve weeks.

Considering these facts if you are stable and you want to stop the olanzapine just stop the injections.

Answers:
1. Any type of Nicotine intake either smoking ,chewing , in lip or nose increases metabolism of Olanzapine by inducing hepatic CYP1A2 enzyme.So overall effect of olanzepine will be reduced.It is difficult to calculate that at which rate metabolism will be affected. As it depends on various factors like age, dose of olanzapine, nicotine intake, genetics etc.But when you want to stop olanzapine no need to increase use of nicotine.

2.Answer of this question is explained in initial explanation. It may take 8-12 weeks to clear olanzapine from body from last dose of injection.

3.You have taken last injection before 34 days. You are thinking to start 5 mg medicines. It means you want to take the same equivalent oral dose of injection. So at last you are not reducing the drug at all.
In my opinion wait for 8 weeks. You would not have any problem until than for sure if you are clinically stable since long.After that(or even before) if you feel significant symptoms with severity like anxiety, appetite changes, concentration problems, confusion, crying spells, depersonalization, depression, diarrhea, dizziness, fatigue, headaches, insomnia, irritability, memory problems,mood swings, muscle cramps, nausea, panic attacks, psychosis, restlessness, suicidal thinking,sweating, vomiting, weight loss ...: you can start 2.5 mg dose and gradually stop it after doing half after 2 weeks.

Discuss above aspect with your treating doctor and move forward to stop olanzapine under medical supervision..

Note: In most of the cases of confirmed schizophrenia low dose of antipsychotic should be continued for long.So before stopping the drug completely discuss this issue with your doctor.
