Question: Hi. Lately I have had a ongoing pain in my upper back that just isn't going away. What medically could this type of symptom be? I of course am overweight. Take a Thyroid pill and I smoke. No gaul bladder and I
am going to be 56 this year.

The back/spine needs to be examined. MRI of the dorsal spine can be advised if relevant on examination.

ECG, Blood counts, x ray dorsal spine, ESR can be done prior to the above MRI.

GERD often presents with epigastric discomfort and /or upper back pain. PPIS can be advised empirically for this purpose to see for improvement.
