Suggest Treatment For Tinnitus Causing Sleeping Difficulty

Question: Doctors, reading below questions and answers from a 70 year old tinnitus sufferer (man) are on target for my question. I have NOT be able to sleep for the past 36 hours due to the volume level of the tinnitus I'm suffering. This particular (loud) tinnitus started 15 minutes after trying to go to sleep and hasn't stopped since then. The tinnitus came immediately and right out of the blue - bam !! I couldn't get into my ENT so had to "visit" the ER. They put me on 875mg Augmentin (infected throat), but didn't offer any remedy(ies) for the loud tinnitus. To reiterate, I can't sleep . . . would 5mg of Ambien (or stronger, i.e., 10mg or 15mg) help me get to sleep ?? I've reached the panic state due to sleep loss. Beyond Ambien, are there any other immediate resolutions to my sleep issue ?? Please advise the soonest. Thank you.

Hi, thanks for your query.

In my opinion, ambien tablet 10 mg can be taken to help you sleep, if you been taking it in the past. If it's the first time, I recommend you see a doctor before using it.
I would not user Diazepam tablet. Get your BP checked. High BP can cause loud tinnitus.

If I were your treating doctor, I would have prescribed ginkgo biloba tablet 4 0mg twice a day and
betahistine 24mg twice a day for 15 days in addition to the antibiotic. A short course of steroids may have been helpful too.

If feasible try visiting an ENT surgeon later. You might need an audiometry test to know about the auditory nerve status.

Salt restricted diet is important.

Dear Dr. Deepashree,

Thank you for your detailed reply. Yes, I have used 5mg Ambien (rarely) over the past many years and have some handy. I'll try that now - with luck !! I do have an appointment scheduled with my ENT/ENT Surgeon on Monday (23 June). I will pose the Ginko Biloba and Betahistine with him as you recommended. I'm not familiar with those meds. I'll also advise him of a short regimen of steroids as you "suggested". Is there any particular steroid you'd particularly suggest ??

As far as my BP is concerned - it was "through the roof" when checked at the ER. I do have HBP and take 10mg of both Lisinopril and Norvasc daily. However, I don't know which came first today, "the chicken or the egg" - in other words, I think the (now long term) loud tinnitus (over this 16 hour period) complicated with the resulting lack of sleep [for over 24 hours] was actually contributing/causing the HBP.

Please advise.

Hi, thanks for your follow up query.
In my opinion, dexamethasone steroid will work wonders. But since you are on high BP, not advisable.
If I were your treating doctor, I would have prescribed deflazacort steroid which have less impact on bp.
High blood pressure can causes tinnitus of any intensity. Your pre existing tinnitus must have been aggravated with increase in bp.
This is disturbing your sleep which again can worsen both brand tinnitus.
I would suggest you to consult doctor and discuss my view with him.
