1.As you have mentioned that you visited a dentist, so kindly explain me what did the dentist advise you, what treatment he did and are you taking any medicine advised by him?

2. Is the tooth decayed?

3. Is the tooth loose or mobile?

4. Are the gums or the cheek swollen..

5.Any other medical history like Diabetes, blood pressure or anything else for which you are on any Medication?

6. Also tell which tooth is it, like upper, lower, front or back tooth??
