Suggest Treatment For Tenderness In Clitoris And Presence Of Smegma

Question: Hello Dr Mazumadar. About 6 days ago I noticed that my clitoris was feeling a bit tender. To give context, I had been masturbating a lot in the previous days, with long finger nails and also using a toy which I suspect was not clean, and I put it down the to that and didn't think a lot more of it. The tenderness persisted and so I lifted up my clitoral hood to take a look. There was A LOT of white smegma, and upon clearing it away I noticed 3 symmetrical round-ish abrasions arrange as follows, one on the clitoris gland itself and two above it to either side, on the underside of the clitoral hood. They were dappled red in the middle and had a vague white outline. More symptomatic info, there was no pain on urine passing over then, no lymphnode enlargement or pain in the groin and the rest of the vulva appeared normal with no swelling or soreness. When I first found them I cleaned away the smegma, and touched them to see if they were painful themselves to the touch. No pain or stinging was felt, just this tenderness on the clitoris generally. In the following days I made to sure to keep cleaning away the smegma and it all healed very well and left no marks behind. On the second day I also applied thrush cream under the clitoral hood which really sped up the healing and took away the soreness. I am very prone to yeast infections and I was wondering if it's possible to get a yeast infection under the clitoris which causes breaking in the skin? In the 2 days since they completely disappeared I have continued to feel extra sensitivity in the clitoral area, although it looks totally normal again. It's not pain as such just an odd sensitivity. I have pictures of the area on the first and second days if that is helpful.

For further context, I was in a monogamous relationship for five and a half years with a man, my most recent sexual partner aside from that was two and a half years ago. It was a female and we engaged in oral sex & vulva to vulva contact. I never had any skin symptoms/ issues in the immediate or close aftermath of that encounter and since then have only had sex with the male partner.
I have had full STI screenings as recently as 2 months ago and everything was negative.
I have only had sex without condoms with two men, both of which I was in monogamous relationships with at the time.

nother thing I forgot to mention, I did experience some heavy discharge around the time of noticing the soreness on the clitoris, but that is not particularly unusual for me as I often have heavy vaginal secretions.

The wounds in the attached images look to me like friction wounds and nothing else, which is good news. The most likely scenario was indeed a yeast infection, and nothing more which is why you responded well enough to the thrush cream. If it were not a fungal/yeast infection then not only would you not have responded well enough to the thrush cream but your symptoms would have still persisted.

I do not see any cause for concern, but I would also like to request a more recent image of the region in speaking, as you seem to have attached images of the region on day 1 and 2 of the infection.

As for the increased sensitivity, it will just take some time for you to return to normal function and sensitivity mainly because the most superficial layer is still recovering from the infection, and the new cells are yet to accustom themselves to their normal level of functioning.

Thank you so much for your thorough reply, it has really put my mind at ease and made me feel a lot better. This has been something that was causing me a lot of anxiety and worry. I was very concerned that I had HSV2 and your reply has made me feel a lot more reassured that it is probably not that.
I think I am still going to get the antibody blood test but I feel more positive that I likely to do not have it now.

As requested I uploaded two pictures of the area that I took today. As you can see it looks totally normal now.

Yes, I always recommend investigations for the sake of the patient, which provide them which a lot more satisfaction and peace of mind, especially when they are worried about some major STDs. So please do not hesitate to go ahead with the investigations. But I am more than positive that you do not have HSV-2.

The latest images report a totally normal looking external female genitalia, and I would not worry at all if I were you.
