Suggest Treatment For Steven Johnson Syndrome

Question: hi doctor
my mother is recently diagnosed with steven johnson syndrome. . her skin and throat is affected with infections. . she has been hospitalised for 4 days and now taking treatment from home. but still she has pain in her throat. her face skin has recovered. she has fever off and on.
can you suggest the best possibke treatment for her in ludhiana (punjab)

1. Since she has fever of and on, get complete blood tests and culture swabs from the skin and throat done. A raised WBC (White Blood Cell) count or a positive culture will help identify if there is any secondary bacterial infection. Blood sugar levels, serum electrolytes. liver and renal function tests are important. Get her Chest-X-ray and ECG done.

2. She needs pain relief by either medicated mouth washes, painkiller or local steroids. There is no specific treatment for SJS (Steven Johnson's Syndrome). You have not mentioned if this was triggered off by any medication or infection. All earlier medications should be stopped. What is a her age?

3. It is important that other similar disorders are ruled out and the diagnosis of SJS confirmed, preferably with a skin biopsy.

4. I would recommend that you get an opinion at CMC (Christian Medical College), Ludhiana.

1. The recovery time for SJS varies as per the severity and the factors enumerated above. It can vary from a few weeks in mild cases to months in severe cases.

2. The best treatment in Ludhiana will be available at either CMC, Apollo or Fortis Hospitals.

3. Age above 40 increases the risk of complications. Ensure that she is diagnosed as drug induced or non-drug induced and treated accordingly. i must emphasize that there is no specific treatment and that she needs close monitoring by an MD Medicine and an MD Dermatology specialist. For an exact estimate she will need to be seen regularly. Another option is to upload close-up images here along with all the detailed clinical history and reports advised in the previous answer. Indoor re-admission may be considered in all but the mildest of cases.

1. From the images, it seems to be a moderately severe case. The eyes are closed so the get the eyes checked. The inside of the throat and all exposed mucous membranes in the body, including the genitals should be examined and treated.

2. The lesions see to be healing, which is a good sign. In fact the treatment is similar to a case of thermal burns.

2. If the lesions cover more than 10% of the entire body surface area, it may be a more sever form of SJS known as TEN (Toxic Epidermal Necrolysis). That is why she needs a full body examination.
