Suggest Treatment For Social Anxiety When Suffering From OCD

Question: Hi Doctor,
I feel conscious while talking to people and my eyes always goes to private part of people and its always a very awkward situation for me as well the people who is in front of me and believe me that there no wrong intension behind it. It happens with both male and female. If private parts is not visible to me , I feel normal and behave normal till some extent otherwise my mind always stuck in thoughts not to see that parts and my mind and my eyes are not in control of me and goes to that area only and I always feel afraid that people are observing me and I feel guilty.

Thoughts continuously comes in my mind for not see that parts and I feel like block in a loop and try to avoid that situation.

I am working women and try to avoid talking to people because of this condition. I have tried to avoid calling people at my home and always avoid to meet people so that I can avoid this condition.

I appreciate your efforts for medical consultation in so much distress.

Important aspects of your query are:
•     33 year old female
•     Known case of Obsessive Compulsive Disorder (OCD)
•     History of Poor response to treatment form psychiatrist and from Ayurvedic doctor.
•     Complaints of: Severe OCD symptoms and depression.

From history it is clear that you are having problem of OCD predominantly in form sexual obsession and compulsion. OCD cause severe distress and anxiety to the person. Due to that untreated or poorly treated patient may develop depression like in your case.

In my clinical experience ayurveda and homeopath treatment do not have good response in OCD. I see many patient who come after taking such treatment.

You have already have some benefit, as you are bit relaxed now. Many patient take time to show good response on treatment and duration differ 1 to 3 months to show response.

Some time patient (40% cases) did not improve with initial treatment, in that case one of the following steps are used:
-To rule out any medical condition leading to existing symptoms or causing treatment resistance.
-Detection and correction of vitamin deficiency (especially vitamin B12)
-Screening of thyroid dysfunction: Get serum T3/T4 and TSH level done.
-Checking compliance to treatment: Take medicine regularly: as medicine used to show effect after 4-8 weeks of continuous use.
-Increase the dose of current medicines
-Change of existing medicine
-Addition of other medicine
-Addition of psychotherapy like cognitive behaviour therapy (CBT) and relaxation therapy in treatment.

Usually SSRIs (Fluoxetine, fluoxamine) and TCAs (clomipramine) are treatment of choice for OCD. Medicines are prescription medicines, which are not allowed to advice on online platform. As you are in XXXXXXX NCR, you can meet me personally for treatment.

Hi Doctor,

Thanks for your advice. I am a mother of 2+ year onld child so currently it is not possible for me to go out of XXXXXXX for treatment on regular basis. Could you please provide me online consultation if possible and can provide benefit in my case?
I am not taking any medicines right now and even don't feel relaxed, I am always get irritate on my family members as my mind don't feel relaxed and feels exhausted.

If I am suffering from OCD predominantly in form sexual obsession and compulsion. What is the treatment of this disease and how long it need to be continued?

Also suggest how could I go for psychotherapy like cognitive behavior therapy (CBT) and relaxation therapy in treatment.

Is it possible online? or I have to meet you personally for therapies? or suggest me any option if available in Noida?

I can understand your concern but, for prescription you need to meet nearby psychiatrist.

I am not sure whether the doctors name sent by you are expert in CBT. I will confirm it from them.

I just want to know one more thing. Medicines for treatment of OCD include any sleeping pills? as I am a working women with very busy schedule of 9-10 working hrs/per day . Do I need to take any break during medication or can I continue my work routine as it is.

OCD medicines are not sleeping pills, but in 10-20% cases treatment may have some sedative effect for 1-2 weeks, which can be corrected by drug dose optimization. Medicines can be continued with regular job.
