Suggest Treatment For Sleeplessness

Question: Hi Doctor,

Last time you helped me to rule out cardiac issues and hypertension, since I am not suffering from either of them, I do feel anxious at times and at times when my brain is blank I do fall asleep and manage to sleep for 6-7 hrs, but at times when I go to sleep my brain keep thinking something or the other and thereby the sleep is affected I cannot sleep, but when I fall asleep no issues, its just that I need to fall asleep, my nature of job is 24/7 which means I need to work at night at times and at times during day, so switiching from day to night and vice versa becomes difficult and at times I dont sleep for whole day due to this.

My BP readings pretty much all the times are as below :

115/68
118/74
119/76
126/78
133/80
109/68
121/78
120/74

Heart rate always is 60 - 85, during stress or panic it tends to be 90 or may be even 95, but generally it stays 60 - 75 or may be 85.

Had a stress test 6 months ago, marked negative for ishaemia, had an ECG which came back within normal limits, Had an echo as well it was excellent.


I generally take multiple readings while measuring blood pressure so above are the figures, the figures above are just always like that I means it has never gone above 135 at home systolic and above 85 at home Diastolic.

I do suffer from Mild asthma but not taking any medication for it, apart from ventorlin inhaler, I take inhaler when fel asthmatic, I use is once in a day or may be twice at XXXXXXX My concern here is :

1) What may be causing the sleep problem, remember Once I fall asleep its ok, its just that when initially trying to sleep it becomes difficult, my brain thinks a lot.
During my sleeplessness I monitor my BP and heart rate, they are excellent when I check during sleep, they are 109/68 and the heart rate is 60 - 69, so I am not feeling any congestiveness or anything, its just the sleep, once sleep all good upto 6 - 7 hrs.

2) What do the above readings suggests, do I have BP ? My physician had provided me with atenolol considering the BP at his office, but whenever I check my BP it has never crossed 130 is systolic and 85 in diastolic at home, it stays below 125 systolic and 85 diastolic without taking any Aten, average reading for me ranges from 114 to 118 sys/65 to 78 diastolic, occassionally it can go upto 130/85 but not always.

Welcome back. I remember you well. You had issues about feeling of pulses in your feet, which i had explained to you, that is absolutely normal.
Now, following are your answers:

1. You are unable to induce sleep, and that is mostly because you are anxious. You must be thinking something or the other, while you are at bed trying to sleep. You might also be monitoring your BP and heart rate while you are trying to sleep. So, that stress and over thinking causes difficulty in sleep induction. So, this is not a disease. It's just you need to relax. I would advise you not to sleep at all in day time in case you do, because that can cause difficulty in sleep induction in night. Avoid taking coffee, cold drinks, tea before you sleep in nigh. In fact you should avoid taking such beverages after 7 pm. You should try to meditate before you sleep. Start exercising. Work hard, make your body feel tired. You will surely get your sleep induced easily in night.
In severe cases, we use Zolpidem for short course for sleep induction. But in your case, i would advise you just to follow the above mentioned advise.

2. No, you don't have hypertension. You all BP readings and pulse rate are absolutely with in normal limits. There is no indication to treat you. You just need to observe low salt and low fat diet. I would also advice you again to stop measuring your BP and Pulse again and again. As a part of routine checkup, you can check it once a week.

I don't find any issues with you. You are just little anxious.
