Question: Female age 77. Many skin freckles forever. Am not a sun worshiper. Always use sunblock. Now, large dark brown spots have been appearing for several years and are becoming larger and more frequent. Thumbnail size and larger. The spots are dryish. Mostly on face. Treatment to reduce the visual impact of the spots?

I am Dr V XXXXXXX

I will help you in resolving the skin problem.I deal with good number of elderly patients having similar problem and needing a safe treatment.

Here are the measures with good potential that should lead to improvement in the coming weeks and months.

1 Retino-A or tretinoin containing preparation in 0.025% concentration is a very effective preparation for this purpose. The method of application has to ensure that for he first few days, say5-7 days, one should apply it in very thin layer directly on to the area to be treated for a duration of about one hour after 7 pm. It should be cleared after this period with just plain water face wash.

Thereafter, the duration of application can be increase by 15 minutes every week up to three hours.
If there is any irritation, then one should stop use for two days, and restart using slightly reduced duration of application.

It generally shows results in about six to eight weeks in most of my patients.

2 Anti-oxidants by mouth help in facilitating recovery

3 Sunscreens must be worn inside as well as outside home or buildings to prevent dark spots
