Question: Male 53 yo with severe itching near top of butt crack. Has already had hemmeroidectomy. Negative for pin worms. Bloody and cracked and wakes him up at night. Cortisone cream did not help.

Hi
As per your description, it looks like Fissure in ano.
It is a crack at the edge of the anus,which sometimes bleed.
Stool should be soft. Please drink plenty of liquids, eat fresh fruits and vegetables.
I prescribe Ointment containing Diltiazem to my patients with similar complaints.
If there is no response with above measures, Surgery(Anal dilatation or Lateral sphincterectomy ) has to be done .It is a simple day case procedure.

Thank you for the response. However, it is not really at the edge of the anus that itches severely. It is more the other end of the crack. There are bloody, small cracks and the itching is unbearable.

Hi
It could be a Sentinel tag, a fold of skin at the edge of Anal opening. Inner aspect will have cracks and will be bloody. Later, mucus gets secreted and becomes hardened leading to itching.

Please follow dietary measures and use local Diltiazem gel.

If there is no response, please consult your Surgeon and proceed.
