Brief Answer:
consider adding a topical steroid and antibiotic cream

Detailed Answer:
HI, thanks for using healthcare magic

Though a fungal infection is the most common reason for a rash between the thighs because of the heat and moisture, it would have been expected to improve after 3 weeks of persistent use of an antifungal.

You should consider adding a steroid cream such as hydrocortisone along with a topical antibiotic ((eg bactroban, fucidin, neosporin) as well. Itching of the skin can cause bacterial under the nails and on the skin surface to enter the skin which can cause bacterial infection

Summary: (1) add a topical antibiotic and topical steroid cream to the anti fungal
Use twice daily.
The steroid cream should be used for 5 to 7 days only.

I hope this helps, feel free to ask any other questions
