Suggest Treatment For Severe UTI

Question: hello, I have just returned from my gp , feeling confussed and worried. I am a 37 year old female with a large family history of gyn cancers and breast cancers ( my mum has secondary breast cancer) , this has made me very cautious of any new problems that arise. Last weekend I ended up in a&e after having awful awful pelvic pain, they put it down to constipation and sent me home, however the next day again I was in agony , so went to gp who found pus and blood cells in urine , they sent off a sample and gave me antibiotics, 24 hours later the pain had gone from a 9 to a 2 , which was great , but I still wasn't using the toilet properly , only small amounts of soft stools but pressure on my lower tailbone and bladder and bowel . it hurts when I cough and hurt when I pass gas. I went back to gp who said I had white blood cells in my urine and to go back after I finished antibiotics and I would do another sample and also a vaginal examination. I asked if he thought it was anything serious and he said he cant rule out anything serious out yet but I shouldn't worry . last year I had a cervical smear all ok ( previous history of abnormal smears and I had cells lasered once) also I had a c125 ovarian cancer blood test and that was ok. I also had a ultra sound scan and they found a small lump but said it was too small to be classed as a cyst ( previous history of ovarian cysts also which went on their own). but this is increasingly worrying now , could something have been missed , iv had irregular periods for 18 months now ? , by irregular I mean sometimes they are on time some are late sometimes iv missed a month. iv not noticed they have become heavier or more painful , please advise me on what the doctor could have meant by he cant rule out anything serious. and do you suggest I pay private to get a diagnosis . or could all these symtoms be a sign of uti, ill brief , constipation , pressure , lower back pain that makes my legs feel strange, frequent urination, rectum pain, bloating( pain in upper shoulders when iv eaten a meal) , tierd and feeling not right . I also have 2 children 13 and 8

thought I should add im going on holidays for 2 weeks tomorrow so that's why I needed your advice before I go and gp said I should not worry and enjoy my holidays

It nice to see such an elaborate history firstly. The fact that you have been diagnosed with white and red blood cells in urine culture simply indicates that you are suffering from a severer or chronic urinary tract infection or cystitis, which needs an antibiotic treatment for a full one week and then a repeat culture. If the repeat culture is still positive then the course of antibiotics can be extended for another week.
A UTI can present with increased frequency, urgency, burning micturition
, lower abdominal pain, lower back pain extending to inner thighs and generalized weakness, mood swings and even decreased appetite. There can also be a possibility of a pelvic inflammatory disease if the pain is on both pelvic areas and continuously dull aching and not relieved by natural methods but NSAIDS. This would be confirmed only after a per vaginal examination if there is cervical motion tenderness. A high vaginal swab may be taken for culture and a repeat PAP smear as routine again. In case of PID , again antibiotics course for 2 weeks would be an ideal treatment.
This seems like a UTI only and you need to complete the course of medications before complete relief can be achieved. Increase your water intake to 5 liters a day as this would allow early flushing out of infection. You may try cranberry juice , one glass a day, which shall alkalinize the urine and remove infection sooner.

thank you for your medical opinion. I will ceetainly follow your instructions . and hope that it is just such a uti . it was my own gp's words that unsettled me and due to the high family history and previous gyne problems it certainly unnerved me. thanks you

oh just to verify would my bowels be affected by this as I am still having pressure and spasms in rectum and not having normal bowel movements again as yet

Hello,
The bowels tend to get affected in cases of pelvic inflammatory disease or endometriosis usually but it can happen in severe UTI as well when rectal irritation is possible. The antibiotics would control the PID as well along with UTI and you should be benefitted in a week's time. Just take the medication regularly and then the repeat visit to your physician for a repeat culture and gyno exam can decide further course of action.
By en large, this doesn't appear to be any cancerous presentation as there is no weight loss or severe decrease in appetite or hematuria per se. So relax and enjoy your holidays.
