before 2 months ago i have put a question about intching in private parts

and doctor suggest me as below

Hello,

Thanks for writing to health care magic.

It seems you are having most probably fungal infection of genitals.In medical terms this is known as tinea cruris.

Body parts such as thighs,genitals ,arms are always moist and prone for fungal infections.Using tight undergarments,fit clothes,excess work in hot weather can also lead to fungal infections.

It will always better to keep area dry and clean.Use loose cotton undergarments.Avoid use of jeans,and if you are a outdoor worker I will advise you to bath twice in a day.

Creams which you are applying probably contain steroids.It will reduce only itching and redness,but not fungus.It will be better if you stop these creams and go for anti fungal powders like candid.This will also help in keeping area dry and prevent relapse.

However mainstay for fungal treatment is oral antifungal drugs.In my opinion A course of oral terbinafine 250 mg for 20 days will help in cure of the disease.

Antiallergic tablets like cetrizine will also be required for control of itching.

I would keep a possibility of Recurrent Tinea Cruris.

From your history I could gather that you were asked to take an oral antifungal i.e Tab Terbinafine 250 mg, an oral antihistamine (Cetrizine) and antifungal dusting powder.

-I would like to know whether you applied a topical cream? If yes, let me know.
-What was the response to the above treatment? Did it improve and then recurred Or it never improved in first place?

Tinea Cruris can be recurrent. The possible reasons being:

--The presence of predisposing factors(underlying diabetes;Obese individuals; hot,humid condition etc)

--If the treatment is taken irregularly or incompletely(most likely factor in your case), regular treatment last for 4- 6 weeks.

--Sometimes due to constant scratching an element of eczema sets in and the skin becomes lichenified (thickened) which itself is an itchy condition and thus it becomes a cycle, the more you scratch the more it itches and the more is the desire to scratch (Itch Scratch Cycle)

--Sometimes if the fungus is resistant to the oral antifungal. Azoles like Fluconazole, Clotrimazole are fungistatic (inhibits fungal growth) therefore it does'nt kill the fungus; that may be one of the factors for recurrence especially in hot, humid conditions when the fungus gets a chance to re-proliferate.
