Brief Answer:
Pimple formation is basically due to hormones

Detailed Answer:
Hi XXXXXXX

Welcome again and thanks for posting your details with pictures.

I can understand your concern for pimples. It is really bothersome to have something unwanted present on our face.

Before I proceed to your issue let me explain you elaborately regarding pimples-

Acne or pimples in most of teenage people starts appearing at the age of 13-14 years and then almost vanishes by the age of 25-30 years.

The main cause of pimple formation is basically due to hormones (chemicals which are influencing one or other body functions) in addition to the influence of certain other factors causing its aggravation.

The hormone plays the most important role in high oil secretion and increase the size of oil glands. As the pores or ducts of the glands are blocked so it leads to formation of pimples.

There are certain other factors such as application of oily or creamy products, over exposure to sunlight, heat and humidity or due to pinching, popping and squeezing of the pimples and sometimes over medications also leads to formation of pimples.

Once these are healing they leave the most problematic thing which are either marks or scars.

Therefore any of the above factors along with hormones can cause appearance of pimples before the age of 25 years. After the age of 25 years there is less upsurge in the hormones and these gets stabilized and therefore less appearance of pimples after 25 years.

In some cases of females it may be prolonged till 30-35 years. females have an extra tendency of upsurge and down-surge of hormones in different phases of menstrual cycle. So pimples can erupt more near the menses or any time between the cycles.

So probably you got some idea regarding your acne.

There is no correlation of food with acne and it is just a simple hormonal process only.

Getting one or two pimple once a week is not at all a big issue. The only problem appears when we have marks left after that.

There are certain best of suggestions which you can follow till I suggest you further:

1. Do not pinch, pop or squeeze any area on face. You do not try to even touch the area as is the sole most important cause of aggravation of pimple as well as formation of mark.

2. Do not apply any creamy or oily stuff or any other cosmetics over face.

3. Try to avoid any home remedies suggested by your friends or family members or otherwise self implicated therapies also. Most of he times any of the home remedies blocks more of the pores and results in more aggravation of pimples.

4. Do not take steam or steam bath or massage on your face as these also aggravates the pimples.

5. Try avoiding situations of direct continuous exposure to sunlight or heat and humidity for long hours.

6. Kindly do not use any antibiotic as self medication.

7. Do not apply any hair oil on scalp or apply in very less amount. Wash your scalp daily with some good shampoo and let there be no dandruff in your scalp (because dandruff itself causes blockage of pores of oil glands and then pimples).

I can get that presently the pimple part is quite better and there are only mild grade of spots remaining.

For the pimples I prefer to use a combination cream of Adapalene with Clindamycin over it in a very thin layer so that it should not be visible to the naked eye.

There are certain medication which helps out in reducing the pigmentation on face. If I were your treating dermatologist then would have prescribed a cream (under dermatological supervision) containing Kojic acid with arbutin and mulberry extract in it.

Alternatively Azelaic acid gel can be applied over the affected area of pigmentation. These are one of the best and safest creams to be applied.

I guide my patients to apply any of these creams in a very thin layer at night time only so that these should not be visible to naked eye. Apply at night time before sleeping.

Simultaneously sun protection is a must and a proper gel based sunscreen should be used. In your kind of skin I prefer to use a gel based or water based sunscreen.

Diet is also quite important in reducing the pigmentation. Good diet not only helps in renewing the skin better as well as works best to protect our skin. You are eating proper things in your daily schedule....so do continue the same.

I again reassure you that pimples are not at all disease state and are not at all due to any impurity of blood or any other thing. It si just a part of our life process and will vanish by itself. I have not suggested you any medications to eat just because of the fact that these ar eof very mild grade of acne.

So young lady, just relax and try to follow the suggestions provided above. I hope these information will help you. I will be glad to help you any further.

I will be glad to reply any further queries.

Wish you good glowing skin,

Brief Answer:
Start with low percentage of 10% and then 20%

Detailed Answer:
Hello young lady,

Welcome again to the forum and thanks for your swift response.

In my patients I suggest them to apply azelaic acid (Aziderm 10% Gel) with low percentage of 10% for few weeks and then can increase to 20 % (Aziderm 20% gel) after few weeks.

I suggest my patients to apply in thinnest possible amount so that it should not be visible to the naked eye.

I hope these informations will help you. I will be glad to answer any further queries or if not then can close the discussion and rate the answer.

With regards,

Dr Sanjay
