Suggest Treatment For Persistent Diarrhea, Vertigo And Gastritis

Question: hello Drs,

I have been feeling helpless since last 3 years. My symptoms started with vertigo, hypo thyroid, h-pylori and gastritis. i was treated for for the above and am still on thyroid dose of 50 mcg every day. Lately i have developed serious disassociation (de-realization) especially after taking food. i get disoriented with my surroundings and go in a dream like state / blurry vision. I cant focus and go in a tunnel zone while talking to people. Also sometimes i feel cold and hot (inflammation kind of feeling) / liquid flowing feeling on my head on random spots. I only feel better after passing stool after alot of effort as I m either usually constipated / boated or go to the loo 3 to 4 times a day with diarrhea. Bright lights / crowd / shopping malls and noise bother me a lot. I have been to the neurologist, had mris, eegs done. All reports came out fine. Have been referred to a psychiatrist as well but i know it is somewhere related to my stomach. I am not sure if it is the stomach which is disturbing the brain or vice versa. Please help!!! i'll be grateful.

Have gone through all your details.I am sorry but it sounds like pyschosis . Psychosis is characterized by an impaired relationship with reality. And it is a symptom of serious mental disorders. Psychotic's may have either hallucinations or delusions. Hallucinations are sensory experiences that occur within the absence of an actual stimulus.
Schizophrenia is a form of psychosis,a severe mental disorder in which people interpret reality abnormally. Schizophrenia may result in some combination of hallucinations, delusions, and extremely disordered thinking and behavior that impairs daily functioning, and can be disabling.Schizophrenia is a chronic condition, requiring lifelong treatment.It's not known what causes schizophrenia, but researchers believe that a combination of genetics, brain chemistry and environment contributes to development of the disorder.

Schizophrenia involves a range of problems with thinking (cognition), behavior or emotions. Signs and symptoms may vary, but usually involve delusions, hallucinations or disorganized speech, and reflect an impaired ability to function. Symptoms may include-

-Delusions. These are false beliefs that are not based in reality. For example, " i feel cold and hot (inflammation kind of feeling) / liquid flowing feeling on my head on random spots '.
Delusions occur in most people with schizophrenia.


-Hallucinations. These usually involve seeing or hearing things that don't exist. Yet for the person with schizophrenia, they have the full force and impact of a normal experience. For constipation and loose bowels take a probiotic initially.However i would suggest to to consult a pysciatrist. Medical therapy with Anti psychotics like Haloperidol and Lithium should be started as early as possible.

Don't worry things will start getting better once you are on medications. Its not stomach,Its you Brain.

thanks. i get disoriented but i can tell what's real and what's not. also i do not have impaired speech. it's like pressure on head which comes and goes with brain fog. what else could be this ?

Hi again,
It's not necessary that all symptoms are present in every patient.So not having a impaired speech does not means they are not delusion or hallucinations.

Second possibility is it can be atypical presentation of Migraine with aura.Acephalgic migraine does not present with headache however all other symptoms you are feeling can be an aura of migraine.As migraine is diagnosis of exclusion all tests done will turn up to be normal.

However i would suggest you to go for a in person pyschiatric consultation initially before looking for rare things.

thanks. so what tests should i go through to come to a conclusion? i have been through MRIs and blood work.

There are no specific tests available to rule out the above mentioned diagnosis.
In person physical examination plus a detailed history along with response to medications is the only way to reach a diagnosis.
Frankly what i feel is that your symptoms are related to brain and therapy for brain should be tried initially for atleast 3 months.
You have mentioned in your question that you have seen a pyschiatrist?
What did he adviced you?

nothing. he put me on an anti anxiety medicine for 6 months (cipralex/ librax) but that didn't help me with the brain fog / diassassociation.

Hi again,
Sorry but putting you just on anti anxiety medications will not help.
Initially anti psychotic like Haloperidol in doses ranging from 2.5mg to 10mg as decided by your treating doctor.
Haloperidol should be taken in combination with lithium.
Just taking anti anxiety will not help in pyschosis.
I would suggest you to go for in person examination with a good pyschiatrist.

Start proper treatment you will start feeling better in a month.
