Question: Male 50 years old. Intermittent left flank pain radiating to lower back & down arm.

Hi,
I am Dr Mittal.
I have read your query.
I think I would be able to help you.

The symptoms of the query are quite suggestive of herpetic neuralgia. This syndrome occurs after a bout of chickenpox in elderly age.the virus affect the nerves and thereafter, pain may arise- often late.

The pain killers are usually not effective. I think I would suggest to all my patients 2 options

A. Tab amitryptiline
B. Tab carbamazepine

Initially individually and later in combination if needed.

I recommend the same to you, if you have not already tried them.

Hi
You can call it shingles, that's also a herpetic infection. However, in that a different virus is involved.

In this, your infection is over but the nerve damage persists. So you have the pain due to nerve damage, not due to an active infection.

I therefore suggested just the amitryptiline and carbamazepine and no antiviral agents.

The abdominal complaints could be due to gastritis and a simple antacid should help you. I would suggest rabeprazole 20 mg twice daily. You can discuss with your treating doctor.
