Suggest Treatment For Pain In Abdomen After Upper Endoscopy

Question: I had an upper endoscopy done and mild inflammation was noted in antrum and duodenum. I was having NO pain at this time at all. Biopsies said reactive gastritis/ negative for H pylori. Dr. prescribed Protonix. I took that for 5 weeks. During that 5 weeks, I developed severe stomach pains and stopped digesting any foods. I stopped the PPI. Had a colonoscopy and determined that colon was fine. Pain stopped for about 15 days. After that, additional persistent pain happened. Had a small bowel MRI and all looks normal. Still having pain in abdomen. Pain is constant in lower LEFT rib and Right upper/ Right Mid quadrant. Could the PPI have caused a problem in my stomach? I feel now that my stomach is raw. The pain wakes me up at a night and it hurts me to bend forward making driving difficult. Please advise.

I appreciate your concern

PPIs don't worsen the pain of gastritis or peptic ulcer disease. In fact in severe cases, its practical to use high doses of PPI then the conventional 40 mg once a day regimen.

Zantac or ranitidine is a weaker drug which is an H 2 blocker. PPIs are much better and safer drugs in this regard.

I would advise adding sucralfate and mebeverine with the 40 mg twice a day PPI like pantoprazole for adequate effect.

Serum amylase, ultrasound abdomen, chest, x ray erect abdomen should also be done to look for other causes.
