Suggest Treatment For Low BP, Diabetes And Thyroid

Question: Sir
This is XXXXXX from XXXXXXX We have referred our mother-XXXXXXX to you who is diagnosed with positive Vgkc Antibody-CASPR2 for which she was given five days IVIG injections in the first week of April.Since then, we could not see any sign of improvement but instead she had to be readmitted last week due to low BP. Low platelets and high ammonia and liver enzymes Sgpt and Sgot.
Today she is discharged with all these problems settled.
Presently Doctor has prescribed her following Medicines-medrol, Modalert -100mg,Admenta-5mg,Lacosam100mg. Rifagut400mg along with basic medicines for diabetes thyroid and BP.
Kindly tell us are we on the right track of treatment. What would be the tenure of treatment after 1 month of IVIG injections and what would be the second line of treatment in near future.
Pls guides,it is very painful to see our mother in this condition.

Hello,
I have gone through your question and understand your concerns.
The diagnosis seems to be autoimmune encephalitis.
The role of IVIG is there and should be administered in such cases.
The clinical recovery may take weeks to months. Recovery depends upon extent of initial damage and brain involvement.
Present deterioration seems to be related to drugs(Was any drug like valproate been taken for seizures).
After IVIG, control of blood pressure, diabetes and seizures should be the aim of treatment.
The recovery is gradual and continous follow up is required.
No other treatment available.
