Detailed Answer:
Hello!

Welcome to Healthcare Magic.

I read carefully your query and understand your concern.
There are many possible medical condition that can cause swelling of scrotal sac associated with a heavy feeling.
If this swelling has happen slowly over time and it is painless ,then most common causes are:
-Hydrocele (accumulation of fluid in the scrotum)
-inguinal hernia
-Varicocele (enlarged veins in the scrotum
-Cysts in epididymis
-Hematocele (collection of blood in scrotum )
-Heart failure
Most common causes are benign and treatable but first serious causes should be excluded.
A serious cause of scrotal swelling is cancer of testis. In this case you may have noticed a painless lump in testicles.
You should consult a physician as soon as possible for you.A detailed medical history and physical examination are essential to figure out the possible cause and then order the appropriate investigations to confirm it.
You may need to do an ultrasound to evaluate the content of scrotal sac.

Hope it is helpful to you.
Let me know if you have any other question or doubts to clarify.I will be happy to assist you further.
