Question: Hi, I am 29 years old man, sometimes I feel itching near anus, and on itching it irritates, please advise.

I am Dr. Kakkar. I have gone through your query.

Perianal itching can be due to various causes, commonly Piles or Fissure, Candidal Infection or certain Drugs e.g antibiotics like tetracyclines etc.

The cause needs to be addressed appropriately e,g if you have a mass/feel a swelling at the rim of the anal opening during defecation Or streaks of blood staining stool Or pain during defecation, it suggests that either you have piles or an anal fissure. Constipation is common in people who have piles/fissure. What happens in piles/fissure is that, the anal mucosa gets exposed and there may be discharge which can cause irritation/itching on the surrounding skin.

Candidal infection can also be a cause of perianal pruritus because of sweating and maceration, which gives a chance for candida to proliferate between the folds. You may use an OTC topical antifungal+steroid combination cream e.g clotrin-B cream, twice daily for a few days.

An OTC oral antihistamine e.g cetrizine 10 mg, once daily, would provide you symptomatic relief from itching.
