Suggest Treatment For Infertility

Question: Hello,

I am a 25 year old, Nigerian female (26 years old next month). I have been married for 2.5 years now. I had a miscarriage in August 2012 and had since gotten on birth control. My period is very irregular and does not come monthly unless I am on the pill. It can even be amiss for 6 months at a time. O got off the pill on April 22 of this year and have not seen my full period since. I had a tiny bit of bleeding in the middle of May but that went away overnight. My husband and i have decided to have a baby. Can you please suggest to me medical steps to take in order to make this a reality? I would like to know if i should go see a doctor, an obgyn/ fertility doctor. Im mot sure ifnit is too early but i really want to know if there is anything i can be doing or medicine i should be on to increase my chances?

Spontaneous conception with very irregular periods is unlikely. I feel it is time you need to get help from a competent infertility specialist.
There could be many causes for delayed periods but most probable diagnosis is Polycystic ovarian disease (PCOD).

In this disease, there is hormonal disturbance which causes ovarian troubles and hence menstrual disturbance, high chance of diabetes, gaining weight, infertility as well.

You need to get an Ultrasound of your ovaries and other pelvic organs. In PCOD there will be few or multiple follicular cysts seen. Also hormonal tests of FSH, LH, Estrogen, Progesterone, Androstenedione, Thyroid hormones will be needed.

Once confirmed as PCOD, the treatment is mainly medical treatment with hormonal pills to regularize the cycles. But most important is exercising to lose weight and becoming fit. If exercise is not done then the hormonal imbalance will still be there. Proper diet consisting of low carbohydrates, low fat diet with plenty of fresh fruits and green leafy vegetables is needed. Drink plenty of water. Do daily brisk walking for 40 minutes. Avoid junk food, pastries, colas.
You will also need follicular growth scan to know follicular growth and best time for intercourse.

Is it possible that pcod is not visible during pregnancy? While i was pregnant and did a vaginal ultrasound, they did not see any cysts

Yes it is true that PCOD is not visible during pregnancy. Also the hormonal imbalance can develop later also. The irregular periods are suggestive of a hormonal imbalance that needs treatment.
