Question: What could cause an indentation in the top of the calf muscle of a boy, age 11? He's had it for about a year. No pain.

I have gone through your query ,photo and understand your concern. It may be due to some unnoticed injury leading to adherence of skin to deeper tissue (muscle) . As there is no other symptoms so it does not require any treatment however you can get his MRI of calf to rule out other pathology and to relieve your anxiety.
