Suggest Treatment For Impulsive Control Disorder

Question: I was considering getting some therapy for I guess what you could consider "impulse control" issues, but had a couple of concerns that i was hoping you could address.

A little bit of background. I've historically been considered a non-emotional "hyperational" person (as described by my childhood therapists/psychologists). I dont really have many feelings. I can be bummed out or angry if something bad happens to me, and sometimes feel what i think could be happiness, but mostly i just feel bored.

I guess I am also a misanthrope because generally speaking, people bug me. They can be useful and do have their purposes, but thats about it. I know that kind of sounds hypocritical since i too am a person, but thats just what i think.

Anyway, I tend to have murder fantasies at times. Not sure what else to call them. I imagine killing people (not anyone i know, just random strangers). I plan the whole thing out in my head, how i would lure them, kill them, dispose of the bodies, how it would feel, etc. Lately they have been increasing in frequency and intensity.

Im not particularly bothered by the fantasies (i.e. I dont feel bad or ashamed), but i would like to learn more impulse control so that i never act on them, especially given the increasing frequency/severity, as I greatly value my freedom and do not want to go to jail for the rest of my life and i doubt i posess the skill at this point to get away with such a crime.

My main concern with seeking out therapy is the confidentiality vs mandatory reporting. Considering the nature of my urges/fantasies, im worried i would end up getting involuntarily commited or having my reputation ruined. I put a lot of effort into appearing nice and courteous and i dont want all that effort and self-restraint wasted because a therapist panicked or overreacted.

Basically, my questions are as follows:
1. If i seek psychological treatment, is the therapist obligated to maintain confidentiality or is he allowed tell people what i say/said due to the content.
2. What kind of mental health professional would be best suited to help me with this issue?
3. What kind of treatment, if any even exists, will help with this issue anc will it get rid of the urges/impulses completely?
4. Are there any other tips or suggestions that you think can help me?

I read your query and understand your concerns.

Yes the Psychotherapst is obliged to maintain confidentiality till there is no thereat to life of either you or any other person. Your so called murder fantasies are not directed to anybody in specific and as per my knowledge there is no incidence in last of such ill doing by you.
Although it is slightly subjective issue and much depend on therapist but I must acknowledge you that if I would have been your therapist I would have consider to maintain confidentiality.
So I think with given history your confidentiality can not be breached by therapist.

You need to consult cognitive behaviour therapist as they are most suited for impulse control disorders. If you can not locate one in your city than consult a psychiatrist for management of issues. So the option of psychiatrist and cognitive behaviour therapist is the option.

Both medication and psychotherapy is the option. In medication category mood stabilizer and SSRI group of medication can be used. For psychotherapy CBT is the best option.

Developing new interests and occupying yourself is of fundamental importance. Keep yourself busy that can I say at this moment.

Thank you for answering my questions. I have one more question. Does the confidentially extend to past crimes? Hypothetically speaking, if i confessed to commiting a murder in the past, would confidentiality still apply? Or could the psychologist/psychiatrist report me to law enforcement? Thanks.

If you report any crime that therapist is required to law enforcement agencies and law of confidentiality do not apply. In general terms crime is crime no mater when it took place.
