Suggest Treatment For Hypochondriasis And Itchy Skin

Question: i am 37 male.. and diagnosed two tyms with hyponondriasis( anticipatory health anxiety) by a psychiartist and under gone treatment wt veniz xr 75 mg.. got good remission also.. last tym 3 yr bfore my anxiety again returned in the form of a fear towards the onset psychosis ...sply schizophrenia... (early tyms it was hiv infection frmn open space.).
and my psychairtist changed the drig yo prothiaden (dosulepin) 150mg wt topicalm (tofisapam) 100mg two times daily.. this change due to get good sleep tht he told..
bt.. in tht tym aftr four week he stoppd topicalm, on the next moment my anxiety terribly shoot up .. and change its nature from hypocondriasis to primarly obsession.. ( fearing to be accidently engaged in crime , chance to have change in sexual preference... such).. i really fed up...it was chronic.. then i visitd my psychiartst and he reduced prothiaden 150 to 75.. and add new medcine flouxetine 20mg.. .. bt no improvemnt... next visit he upgrade flouxetine to 60mg and reduced prothiaden from 75 to 50.. aftr two weeks i cud feel better... and my obsessional thoughts reduced its intensity.. and remitted 50℅.. anixiety also reduced... then the next visit dr upgraded flouxetine to 80 mg daily and reduced prothiaden to 25 mg.. nw i am going this treatment and i got 60℅ remission in my obsessional thoughts and anxiety... sir my parents are doctors .. general physician.. and they are clearin my doubts... bt in tht crucial anxiety shooted level , onwards i cudnt accept my parents completely... and i have reqmain a question on mind tht.. what if .. if it happend... ..
my anxiety has changed its nature aftr tht... then onwards i cud loss my rational thinking and accepting ability. bt nw a days it slowly return.. i can understand almost things... and in the tym of severe level anxiety, my parents assured tht i never be a psychotic level and all tym i keep and controll my anxiety from others and society.. i am sure,tht even in the chronic level i didnt lost the reality, only happend some confusion state...

my long essay may be borin u.. bt help me for gettin relief... these are my history... here i need to knw some details cz a psychiartist assurance wll accept by mind completely...

is it ocd , it can be controllable means can i get bettr...
and hv any chance to lead this obsession to delusion or schizophrenia..
in this age have any chance for devlopn schizo... cz i read some article tht scizho is men aftr age 35..

my skin is senstive and sme times its itchy... and that itchnes feels discomfrt me... and when itch nesa feela ,i feard tht it may leads to deluaional parasitosis ideas by my mind,cz i am fearin my mind can think any level...
sir this fear of delusional para may leads to real delusional disorders?..

i knw its a long essay.. sorry for that.. i am 35 male...with good health..no famly history of psychosis and severe mental disorder.. no childhood trauma... parents are doctors.. and father has also a little bit anxious bt in controllable level... thanku sir

Hello XXXX,

Thanks for writing to us. I can understand your concern.

You have very well narrated your history. It is clear that you have anxiety problem only, not psychotic. You had obsessional thoughts which are under control with help of medicines. Now your concern about having schizophrenia or loosing reality is illness anxiety disorder only. In schizophrenia some prodromal symptoms are there which psychiatrist can easily identify. In your long history there are no such symptoms. If at all there were chance, you should have schizophrenia before this.I have never seen any patient with such history who turned out in to schizophrenia so there is no possibility of schizophrenia in your case.
You should also consult dermatologist for itching problem. If no reason found out, it can be due to anxiety.
You should continue medicines as per your doctor's advice as well as there is need of CBT - Cognitive behavior therapy. It is a type of psychotherapy and very useful in most of the anxiety spectrum disorder including OCD. It may cure your problem and you can stop medicines in future without discomfort. In CBT psychiatrist/psychologist helps person to identify his/her automatic thoughts and faulty behavior which would ultimately be modified by person with active efforts and help of therapist. It also includes time management, daily sleep, meals and exercise timing. Ask your doctor for the same.

thanku sir... i am goin to conclude.. the one and only doubt is need to clear, my itchin skin anxiety or obsession have any chance to convert to real delusional parasitosis... and is schizo rare aftr age 35... sir i am going to do cbt and i am ready to have use drug for lyf tym..., if i have ise drug lifetime, there has any chance on me to worse my anxiety in current level to severe tht i hav faced when medcine stopd ealier... i really fearin such state... thanku sir....

Hi,

Thanks for follow up.

Delusional parasitosis occurs in old age people and I have seen almost all the patients above age of 50. And itching is less common presenting symptom of the same. Considering your overall history there is no chance that your problem can be converted in to delusional parasitosis.

To keep anxiety disorder in control you must continue follow up regularly with your psychiatrist. Never stop any drug cold turkey. There are negligible chances that you need to take the drug lifetime but I really appreciate your attitude that you are ready for the same. Many patients cured after combinations of psychotherapy and drugs. I hope you will be one of them.
Considering your profile there are very less chances that your anxiety can worsen. Just keep faith in yourself, your doctor and the God and try to think positively actively.
