Question: I am a 50 year old male. I do not have a problem getting and sustaining a erection during sexual intercourse
but just lately I am having trouble having a climax and emitting semen. what would be the cause of this
problem?

By difficulty to have climax or emitting semen, do you mean the discharge is delayed or totally absent? It can be either delayed ejaculation or no ejaculation The later can be a true anejaculation( total absence ) or retrograde ejaculation ( semen is emitted in urinary bladder instead of outside) It can be due to ageing, Diabetes, Urinary tract problems like enlargement of prostate or side effects of medicines like Prostate medicines, Antidepressants and many others You have to detail your problem more to reach to a conclusion

The difficulty is no ejaculation. I am taking a prostate medicine for a enlarged
prostate. I have been diagnosed with prostate cancer with a XXXXXXX score of
six. After a biopsy and further testing Johns XXXXXXX says there is a 96% probability that it will not affect my life style during my life expectancy.

I imagine you are taking alpha reductase inhibitor deutesteride with alfuzocine or a similar medicine. The most common side effect is absent ejaculation or retrograde ejaculation. You can check retrograde ejaculation by finding sperms in post coital urine . The treatment can be quite a difficult job but medicines like pseudo ephedrine may help You can also use ring vibrator on penis while doing sex . This provides direct penile stimulation . Using dopamine agonists like cabergoline may also help The crux is adequate direct penile stimulation . but you must try anything under guidance of your urologist only
