There is no such things that mediines should be taken always.
As the blood sugars are variable, each times medicines will change if the blood sugars are not in control.
But in type 2 diabetes the basic problem is insulin resistance, so metformin is recommended always if there is no allergies and contraindications.
If there is deficiency in the insulin secretion then secretogogues or insulin is recommended..
So for most of the times the recommended drug is metformin, 500 mg usually started once a day and then gradually titrated upto 1000 mg two times a day.

Diabetes is the cause for one of the micro vascular complications which involves kidney, nerves and eyes.
You can delay the progression of nephropathy, but you cannot stop it.
By keeping good condition of sugars and blood pressure you can postpone the progression of nephropathy
Monitoring serum creatinine and urine microalbumin will help in early detection
Having one kidney definately increases the load of work, which can accelerate the progression but dont worry contro sugars and blood pressure.
