Suggest Treatment For Depression, Lack Of Concentration And Chest Pain

I am XXXXXX, a Software professional.
I live in Gurgaon. I am an attention seeker. I am suffering depression due to which I feel lack of concentration, absent mindedness, severe pain in heart. I have been like this from time to time, mostly because I could not impress the girls I like in matrimony sites. When I fall for a girl. I feel heaven on earth and I am the most happiest person on earth. I form a very strong attraction and longing for some girls. Once they say no, I feel very dejected and depressed. Though they dont see me as a bad person I tend to develop a depressive feeling that I could not be attractive, the way they want. I feel do not have any originality , I try to mend myself to their tastes and desire. At last, they tell that I am so sweet, we can be friends, which hurts me severely.
Please help me come out of this depressive feeling and lead a positive life. Help me identify my true self.

I appreciate your efforts for medical consultation in so much distress.

Important aspects of your query are:
•     36 year old male
•     Complaints of: attention seeker, lack of concentration, absent mindedness, severe pain in heart, ideas of worthlessness (could not be attractive), lack of confidence and low mood.

From your query it is apparent that you are have chronic anxiety and depression along with introvert personality. Chest pain can occur in depression due to muscle spasm, but it is better to rule out cardiac illness, after examination and investigations like ECG & 2-D Echo.

There is need for treatment, if symptoms cause significant personal distress or affect personal (not able to get marry)/professional life (poor concentration) like in your case. Both drug and non drug treatment like cognitive behaviour therapy (form of psychotherapy) are used for treatment. Choice of treatment depends on severity of symptoms, personal preference and availability of treatment. In my personal opinion, considering long term suffering combined treatment (drug with counselling) will be best for you.

Anxiety and depression occur due to neurotransmitters imbalance in brain most commonly involving serotonin neurotransmitter. In drug treatment two types of medicine used. For immediate relief of distress benzodiazepines group of medicines are used and for long term relief most commonly SSRI (selective serotonin reuptake inhibitors) are used. SSRI takes around 3-4 weeks to show its effect. Non drug treatment like cognitive behaviour therapy (form of psychotherapy) also takes weeks to show its effect. All these medicines are prescription medicine and given after in person examination by psychiatrist.
