Question: I am male, 78 years old, I have diabetes, I have a heart transplant in 2005, I am experiencing a problem eliminating. I have to strain all the time to accomplish a BM. I frequently feel the pressure to do BM, Feces look like little worms, sometimes I pick up blood when wiping (Hemmoroids?). last two days, I notice very little volume urinating. (kidney?)...

Thanks for the query.

Your symptoms of constipation and less urine output suggest that most likely it is due to less water intake, however there can be various medical causes for low urine output which can include renal failure, urine infection, kidney stones, use of diuretics, etc.

I would advise you to consume plenty of water in next couple of days and see the urine output. In case it improves, then it confirms the diagnosis. However if urine output doesn't improve, then you are advised to consult your doctor for proper examination and relevant investigations.

Constipation is a very common problem in people above the age of 65 years and is generally due to less water intake coupled with weak intestines, less body activity, less amount of food, use of certain medications like antacids,painkillers etc.
I suggest you take plenty of water,do regular exercises and eat well.
In addition you can take LACTULOSE 15-20 ml in the night which wold help in your BM.
This is perfectly safe laxative for diabetics.

Blood in stool cold be due to anal fissure or hemorrhoids and need to be evaluated.
It should resolve once you start having normal stool.
