Question: Uterine cavity normal in shape and size. no filling defect noted.

Both fallopian tubes visualised in their entirety shows mild contour irregularity with dilated distal ends.

Though spillage is noted on right side, no free spillage seen on left side.

>> BILATERAL DISTAL HYDROSALPINX WITH LEFT FIMBRIAL BLOCK

Can this be treated medicinally ? what should be the line of treatment?

Best treatment for bilateral distal hydrosalpinx with ONE sided fimbrial block is SURGICAL correction of fallopian tubes. It CAN'T be treated medically.

It can be done with open up the distally blocked tubes (salpingostomy) & remove adhesion (adhesiolysis). However, infection or PID should be controlled for better result and to increase pregnancy rate. Surgical interventions can be done by laparotomy or laparoscopy.

Women who want to conceive, IVF has now become the major treatment for hydrosalpinx. Kindly, consult with your gynecologist regarding this.

what will be the chances of concieving through ivf if i want to get the left one clipped.. Is clipping a reversible procedure?with laparscopy, is there a chance of relapse of hydrosalpinx?

Hello,

Thanks for follow up query.

1) Several studies have shown that IVF patients with untreated hydrosalpinx have LOWER conception rates than controls. Therefore, hydrosalpinx should be removed or clipped (prior to IVF) to increase pregnancy rate.

2) Tubal clipping is a reversible procedure and pregnancy rate up to 85% after that. It is best choice in this case.

3) It should be done by an operative laparoscopy and it is supposed to improve IVF pregnancy rate because the hydrosalpinx fluid is toxic and can prevent the embryos from implanting. Relapse rate is very minimal.
