Suggest Treatment For Autoimmune Disorder During Pregnancy

Question: Hi. My wife is suffering from an auto immune disorder. Doctor has still not confirmed the diagnosis of an SLE or RH. She has been prescribed with hcqs 200mg qd, folitrax 15mg weekly, folvite 5mg tiw, calcirol and nucoxia 90mg sos.

We are planning to have a baby and delivery in Sep 2016. What are the medicines that should be discontinued. and if discontinued, what should be its replacement now and during pregnancy? She is a non smoker, occasional drinker. She is slim with wt. 49kgs and ht. of 5'-3".

LEt me explain about the medications one by one -
1. HCQ - It is considered relatively safe for use during pregnancy and can be continued.

2. Folitrax - This drug has to be stopped during pregnancy, as it causes gross fetal defects and pregnancy risks.
3. Folvite 5 mg should be continued, and in fact, it is recommended to be taken during pregnancy as it is a necessary vitamin for fetal growth.
4. Calcirol also can be continued during pregnancy as it is a vitamin.
5. Nucoxia 90 mg - It cannot be continued during pregnancy.
SO basically during pregnancy, HCQ can be taken, and if the disease is very severe, steroids can be added .
Auto immune diseases behave unpredictably during pregnancy, and there might be remission or flare up of the disease.
Accordingly, the treatment will be tailored by the Rheumatologist .
The medications should be modified 6 weeks before planning a baby.
