Brief Answer:
Further testing is needed.

Detailed Answer:
Hello XXXX and welcome to HCM.
As an Urologist, i would advise you to do, sonography of abdomen and pelvis ,as well as doppler scan of scrotum, with a TRUS.(trans rectal ultrasound scan).
This will give a detailed information, regarding changes in abdomen and testis, and if there's any pathology causing painful ejaculation and pain in anal region.
Get examined by a surgeon or preferably, an Urologist.
After seeing the reports, a proper treatment will be advised.
Till the reports are available, start an anti-inflammatory drug like serratio-peptidase.
Stop masturbation till reports are ready and seen by the specialist.
Wish you a speedy recovery.

Brief Answer:
Doctors are the most approachable people in society.

Detailed Answer:
Hi again.
There's no need to feel tense at all. You can express any anxiety freely to any doctor.
As the testicular injury was over a year ago, it's very likely, that reports are normal.
Fertility issue can be checked easily,by doing a semen analysis.
As you were hit on only one testis, the other testis being normal, sperms will remain in the normal range.
So, you're worrying without any reason.
Cheer up and just relax. To get out of tension, see your doctor.
Wish you well.
