Question: Hello my name is XXXX. I got my tubes tied in 2007. Im having heavy periods that last 7-2 weeks some time a month. I'm having blood clots that are big as gulf ball and some time bigger. I have to push them out. I have pain so bad in my stomach and back to I can't even walk. I get weak and dizzy a lot. plz tell me what this mean?

Hello,
Thanks for the query to HCM,
The fact that your tubes have been tied actually have no implication on your heavy bleeding or menorrhagia. The most common cause of painful excessive prolonged menses is ADENOMYOSIS or endometriosis. For this you need to get a transvaginal ultrasound pelvis done to look for any endometrial cysts or adhesions and also adenomyosis in the uterine wall.
Secondly another possible cause of heavy menses and clots with irregular shedding is likely progesterone hormone deficiency in the secretory or luteal phase of the cycle. This disallows the growing endometrium under oestrogen influence to mature and hence immature endometrium keeps shedding. You need to get a hormonal profile and especially day 21 serum progesterone levels which if less than 21g/dl shall indicate deficiency you may then have to replaced on progesterone for 6 months during second half of the cycle.
Also required shall be an endometrial biopsy on day 18-24 to look for secretory endometrium of endometrial hyperplasia.In both cases progesterones shall be the treatment.

I have margin were bad that I have stroke like symptoms. what that mean?

hello,
if you intend to mean migraine and dizziness ( stroke like) , then they are both effects of excessive blood loss and possible anaemia . This should be tested with a Hemoglobin test during evaluation.
