Question: I had acl reconstruction in Dec. But my pain and ability to walk stairs is terrible same pain at time of acl injury I cannot squat at all.I had another MRI last week now says trochlear chondrosis with full thickness chondral fissuring.my ortho seems lost, with what to do I have never had any knee injury and am now almost totally debilitated from pre injury life.

Also I was injured on duty in sept 2014 had had surgery in Dec my leg was very atrophied and my pt is very concerned because my acl rehab has not been good because of this.my ortho also said he saw this when he did acl surgery.problem is I feel as bad now a before surgery.my pt thinks my leg is to week to recover from another surgery.

Brief Answer:
Pain is most likely due to chondrosis.

Detailed Answer:
Hello

I have gone through your findings in detail.
Pain is most likely due to chondrosis(degenerative changes in chondral knee cartilage) with full thickness fissure.Findings are most likely related to history of trauma.
Treatment of full thickness fissure is debatable.
You may need arthroscopic surgical intervention as you have severe symptoms.
You may need bedridement and removal of chondral loose bodies.
Osteochondral autografting (mosaicplasty)can also be considered after evaluation. It demands highly skilled surgeon.
You can discuss it with your orthopedician.

It is good that graft of ACL is intact.

Hope I have answered your question.

Brief Answer:
Full chondral fissure is difficult to manage due to limited blood supply.

Detailed Answer:
Hello
Sorry for replying late

Earlier your mentioned that you had injury during duty in sept 2014.Probably findings are related to that past injury.Trochlear chondrosis development in such short duration also indicate traumatic etiology.
Full thickness chondral fissure doesn't respond well to conservative treatment due to limited blood supply to cartilage. So,symptomatic chondral fissure is generally advised for surgical debridement.
